不同材料及球差人工晶状体眼散射比较分析

摘　　要：目的比较负球差疏水性丙烯酸酯人工晶状体眼和零球差亲水性丙烯酸酯人工晶状体眼术后1个月术眼散射情况。方法收集我院行超声乳化吸出联合人工晶状体植入术患者60眼,其中30眼植入负球差疏水性丙烯酸酯人工晶状体,定义为Tecnis ZA9003组;另外30眼植入零球差亲水性丙烯酸酯人工晶状体,定义为Sof Tec HD组。术后1个月使用欧卡斯测量各组客观散射指数(objective scattering index,OSI)、调制传递函数截止空间频率(modulation transfer function cutoff,MTF cutoff)、斯特尔比值(strehl ratio,SR)。使用Pentacam测量各组角膜总高阶像差、球差、水平彗差、垂直彗差,将OSI与其进行相关分析。结果 Tecnis ZA9003组OSI值为2.020±0.693,MTF cutoff值为21.188±5.965,SR值为0.116±0.039;Sof Tec HD组OSI值为1.683±0.624,MTF cutoff值为22.797±7.205,SR值为0.131±0.032,组间比较差异均无统计学意义(P=0.053、0.350、0.131)。两组OSI与其角膜总高阶像差、球差、水平彗差、垂直彗差均没有相关性(均为P>0.05)。结论 Tecnis ZA9003人工晶状体和Sof Tec HD人工晶状体术后1个月全眼OSI没有差异,且与自身角膜像差没有相关性。Objective To compare the ocular scatter between hydrophilic acrylic intraocular lens with negative spherical aberration and hydrophobic acrylic intraocular lens with zero spheric aberration at 1 month after implantation.Methods In this prospective clinical control study,30 eyes were implanted with hydrophilic acrylic intraocular lenses with negative spherical aberration,defined as Tecnis ZA9003 group.30 eyes were implanted with hydrophobic a—crylic intraocular lenses with zero spherical aberration,defined as SofTec HD group.The optical scattering index（OSI）,modulation transfer function cutoff（MTF cutoff） and Strehl Ratio（SR） were measured by OQAS TM Ⅱ at 1 month after implantation.Corneal total high order aberration,spheric aberration,horizontal and vertical coma were measured by Pentaeam.Their correlation with OSI was analyzed.Results OSI,MTF cutoff and SR in Tecnis ZA9003 group were2.020 ±0.693 21.188 ±5.965 and 0.116 ±0.039,respectively,which in SofTec HD group were 1.683 ±0.624 22.797 ±7.205 and 0.131 ±0.032,there was no statistical difference between two groups（P = P.053 P.350 P.131）.There was no correlation of OSI with corneal total high order aberration,spheric aberration,horizontal and vertical coma（all P〉0.05）.Conclusion There is no difference in ocular scatter between Tecnis ZA9003 group and SofTec HD group at 1 month after operation.There is no correlation of OSI with self-corneal aberration.
