摘  要:目的应用Fisher判别函数鉴别诊断模型评价常规超声和超声弹性成像对甲状腺结节良恶性鉴别诊断的价值。方法回顾性分析95例患者共120枚甲状腺结节的常规超声和超声弹性成像图像,以病理结果为金标准,对有统计学意义的超声特征参数进行判别分析,并检验该判别函数模型的鉴别诊断效果。结果通过Fisher判别分析共纳入8个具有统计学意义的特征参数,包括:纵横比、边缘、边界、形态、内部回声、钙化、弹性评分及结节内剪切波速度。回代性检验评价建立的判别函数鉴别诊断模型对甲状腺结节的特异性、敏感性及诊断准确率分别为98.6%、86.3%及93.3%;刀切法评价其特异性、敏感性及诊断准确率分别为98.6%、87.4%及93.3%。结论应用Fisher判别分析法建立的诊断模型有助于克服主观性判断的影响,为甲状腺结节良恶性鉴别诊断提供良好参考,具有较好的临床应用价值。Objective To evaluate the value of conventional ultrasound and ultrasonic elastography in differential diagnosis of benign and malignant thyroid nodules using Fisher discriminant analysis.Methods Retrospectively analyzed of 95 patients with 120 nodules,which conventional ultrasound and ultrasonic elastography images were acquired,according to the pathological results as the gold standard,the ultrasonic characteristic parameters which had statistically significant were analyzed, and the efficacy of discriminant function model was examined.Results Eight statistically significant parameters were enrolled by Fisher discriminant analysis,include aspect ratio,edge,boundary,shape,internal echo,calcification,elasticity score and the shear wave velocity(SWV)within nodules. Using the back substitution test,the specificity,sensitivity and accuracy were 98.6%,86.3%and 93.3% in differential diagnosis of thyroid nodules on the function model of discrimination. Using the jackknife test,the specificity,sensitivity and accuracy were 98.6%,87.4% and 93.3%. Conclusion The application of Fisher discriminant analysis method to establish the diagnosis model can help to overcome the effect of subjective judgment,provide a good reference in differential diagnosis of the thyroid nodules which has a better clinical application value.
