经内镜黏膜下注射及尼龙绳套扎联合高频电凝切除治疗结肠巨大息肉40例疗效观察

摘　　要：目的 探讨经内镜黏膜下注射及尼龙绳套扎联合高频电凝切除结肠巨大息肉的疗效及安全性。方法 治疗组40例为结肠镜检查发现的直径≥2 cm,病理诊断排除恶性肿瘤的结肠巨大息肉,在内镜下先行黏膜下注射1∶10 000肾上腺素生理盐水后,用尼龙绳套扎于息肉根部,再行高频电凝切除;对照组42例单纯使用高频电切除。观察息肉一次性切除率,术中、术后观察有无出血、穿孔等并发症及复发率。结果 治疗组40例息肉均一次性切除,与对照组比较差异有统计学意义（P〈0.01）;治疗组无穿孔、出血并发症,总体并发症发生率与对照组比较差异有统计学意义（P〈0.01）;治疗组6～12个月随访原治疗部位无复发,复发率与对照组比较有显著差异（P〈0.05）。结论 经内镜黏膜下注射及尼龙绳套扎联合高频电凝切除治疗结肠巨大息肉有效、安全。Objective To investigate the efficacy and safety of endoscopic submucosal injection and nylon cord ligation combined with high frequency electric coagulation for the removal of large colorectal polyps. Methods The treatment group 40 cases of diameter greater than or equal to 2 cm giant colon polyps found by colonoscopy which were excluded malignant tumor by the pathological diagnosis was treated with submucosal injection of 1：10 000 adrenaline saline first, nylon rope on the root of the polyp second, high frequency electric coagulation at last. The control group of 42 cases was treated with high-frequency electric coagulation only. The polyp resection rate was observed, and the complications such as bleeding, perforation and recurrence rate were observed. Results In the treatment group, 40 cases of polyps were resected by one time, and there was significant difference between the control group and the control group （P〈0.01）.There were no perforation and bleeding complications in the treatment group. Compared with the control group, there was a significant difference in the incidence of total complication （P〈O.01）. There was no recurrence in the original treatment site for 6-12 months following up, and there was significant difference of recurrence rate between the two groups. Conclusion Endoscopic suhmucosal injection and nylon cord ligation combined with high frequency electric coagulation for the treatment of colorectal polyps is effective and safe.
