摘 要:目的探讨硅油眼继发青光眼房水中自介素-17(IL-17)的表达及其与青光眼发病的关系。方法选择视网膜脱离玻璃体切除硅油填充术后进行硅油取出术60例(62眼)。根据术后是否有继发青光眼分为继发性青光眼组(20例,20眼)和对照组(40例,42眼)。硅油取出术之前抽取两组患者房水标本,以ELISA法分别检测IL-17及其效应分子IL-6和TNF-α的浓度,并进行统计分析。结果前房硅油颗粒、虹膜周边前粘连、硅油乳化等观测指标在继发性青光眼组和对照组之间差异无统计学意义。继发性青光眼组房水中IL-17、IL-6以及TNF—α的浓度分别为(200.45±25.79)pg/ml、(355.86±45.27)pg/ml及(863.27±51.66)pg/ml;明显高于对照组的(150.65±32.57)pg/ml、(238.49±52.16)pg/ml及(622.15±47.13)pg/ml。差异有统计学意义(t=3.75,P=0.04;t=2.86,P=0.04;t=2.15,P=0.03)。结论IL-17在硅油填充术后继发性青光眼患者房水中呈显著高表达,可能通过刺激IL-6、TNF—α等炎症因子的分泌发挥炎症效应等机制参与硅油眼继发青光眼的病理损害过程。Objective To study the expression of IL-17 (Interleukin 17 ) in aqueous humor of secondary glaucoma after silicone oil tamponade. Methods Sixty-two eyes of 60 patients with retinal detachment treated by vitrectomy and silicon oil tamponade and a secondary surgery of silicon oil removal. They were divided into glaucoma group (20 patients, 20 eyes) and control group (40 patients, 42 eyes) according to the intraocular pressure. The aqueous humor specimen were collected from patients in both two groups before the silicon oil removal operation, concentration of IL-17, IL-6 and TNF-α were detected and compared between the two groups. Results There were no statistically significant differences between the two groups in the silicone oil granule in anterior chamber, peripheral anterior synechia of iris or emulsified silicone oil ( P 〉 0.05 ). The concentrations of IL-17, IL-6 and TNF-α were (200.45 ± 25.79 ) pg/ml, ( 355.86 ± 45.27) pg/ml and (863.27 ± 51.66) pg/ml in glaucoma group, which were significantly higher than those in control group (150.65 ± 32.57) pg/ml, (238.49 ± 52. 16) pg/ml and (622.15± 47.13 ) pg/ml (t=3.75, P=0.04; t=2.86, P=0.04; t=2.15, P=0.03). Conclusion High concentration of IL-17 was found in the aqueous humor in the patients of secondary glaucoma after vitrectomy and silicon oil tamponade, it might be the mechanism for IL-17 implicating in the pathogenesis of silicon oil glaucoma, through IL-6 and TNF-ot expression.
