摘  要:目的探讨中枢神经系统感染患者垂体前叶激素水平的变化及临床意义。方法收治中枢神经系统感染患者45例,所有患者入院后行促肾上腺皮质激素(ACTH)、皮质醇、甲状腺功能三项、生长激素(GH)和胰岛素刺激因子-1(IGF-1)、性激素检测;在感染确诊后第3天和感染治愈后1个月,分别检测ACTH、皮质醇、促甲状腺激素(TSH)、游离三甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、GH、IGF-1、泌乳素(PRL)、卵泡刺激素(FSH)、黄体生成素(LH)。观察感染发生前和感染发生后上述激素水平的变化。结果感染急性期出现垂体前叶机能减退者18例(40.00%),其中ACTH、皮质醇水平下降4例(8.89%);FT3、FT4水平下降5例(11.11%);GH水平下降7例(15.56%),5例(11.11%)伴有IGF-1水平下降;PRL水平下降2例(4.44%),其中1例(2.22%)伴有FSH、LH水平下降。感染治愈后1个月,18例患者中激素水平恢复正常16例(88.89%);2例(4.44%)皮质醇水平较感染急性期无明显变化。结论中枢神经系统感染急性期可继发垂体机能减退,感染治愈后多数患者可逆转。Objective To investigate changes and clinical significance of serum anterior pituitary hormone levels in patients with central nervous system infection. Methods Forty - five cases suffering from central nervous system infection at the department of Neurosurgery, Henan Provincial People' s Hospital were enrolled in this study. All of patients received the laboratory tests of adrenocorticotrophic hormone (ACTH), cortisol, thyroid stimulating hormone (TSH), free triiodothyronine (FT3), free tetraiodothyro- nine ( FT4), growth hormone ( GH), insulin - stimulation factor - 1 ( IGF - 1 ), gonadal hormone within 24 h of admission. On the third day post - infection and one month after infection curing, ACTH, cortisol, TSH, FT3, FT4, GH, IGF - 1, prolactin prolactin (PRL) , follicle stimulating hormone (FSH), and lu- teinizing hormone (LH) were determined. Results During the acute phase, 18 patients (40. 00% ) showed apparent anterior pituitary hormone deficiencies, including 4 patients (8.89%) with corticotropic insufficiency, 5 patients (11.11%) with thyroid insufficiency, 7 patients (15.56%) with somatotropic deficiency [5 cases (11.11%) with low IGF - II, and 2 patients (4.44%) with low PRL [one cases (2. 22% ) with low FSH and LH ]. On 18 cases, 16 cases (88.89%) with anterior pituitary hormone defi- ciencies resolved and 2 patients (4.44%) had corticotropic insufficiency one month after infection curing. Conclusion Pituitary insufficiency may occur in a considerable number of patients after acute infection. And the majority may reversible after infection recovery.
