翼状胬肉切除合并干细胞移植复发48例临床分析

摘　　要：目的探讨翼状胬肉切除合并干细胞移植复发的原因及应对措施。方法回顾性分析48例翼状胬肉切除合并干细胞移植复发者的临床资料，所有患者均为第一次行胬肉切除术，且均随访1年。结果48例复发病例中术前血管已达胬肉的进行性边缘13例（27．08％），术后角膜缘有残留组织11例（22．92％），有慢性结膜炎、睑缘炎5例（10．41％），瘢痕体质4例（8．33％），术中移植片紧靠角膜缘5例（10．41％），术中在眼球偏颞侧取移植片4例（8．33％），术后点眼仅10d之内2例（4．17％），术后长时间处于高温高热及粉尘大的环境之中2例（4．17％），不明原因2例（4．17％）。结论翼状胬肉合切除并干细胞移植复发，术前血管已达胬肉的进行性边缘及术后角膜缘有残余组织是主要原因，其次为慢性结膜炎、睑缘炎、瘢痕体质及其他术前、术中、术后的多种因素共同作用造成的。Objective To investigate the causes of recurrent pterygium combined stem cell transplantation and countermeasures. Methods The clinical data of 48 cases of recurrent pterygium combined with stem cell transplantation were retrospectively analyzed, all of the patients were resected for the first time, and were followed up for one year. Results Among the 48 recurrence cases, preoperative blood vessels was useful for margins in 13 cases （27.08%） , postoperative corneal limbus organizers had residual in 11 cases（22.92% ）, with chronic conjunctivitis, blepharitis in 5 cases（ 10. 41% ）, scar in 4 cases（8.33% ）, intraoperative abuts limbal grafts in 5 cases （10. 41% ）, surgery partial temporal side take the eyeball in 4 cases（8.33% ）, postoperative eyes were only ten days for 2 cases（4. 17% ）, post- operative long time under high temperature and high heat and dust environment in 2 cases （4. 17% ）, 2 cases（4. 17% ） by reason unknown. Conclusions Pterygium combined with stem cell transplantation has a relapse, progressive edges of preoperative blood vessels is useful and postoperative corneal limbus have residual tissue is the main reason, followed by chronic conjunctivitis, blepharitis, preoperative, intraoperative and postoperative scar constitution and other due to the combined action of many factors.
