摘 要:目的探讨结直肠癌组织中NF—E1b的表达及其与患者临床病理特征和预后的关系。方法回顾性分析2005-2012年期间在北京大学肿瘤医院胃肠肿瘤中心接受结直肠癌根治性手术的168例患者的临床病理和术后随访资料;男性96例,女性72例;年龄(57.8±11.2)岁;采用免疫组织化学方法检测这组患者的结直肠癌组织标本及其中45例对应的癌旁正常组织中NF-E1b蛋白表达水平;比较不同病理特征患者NF-E1b的表达率.并分析其表达水平与结直肠癌患者预后的关系。结果NF-E1b蛋白表达主要定位于细胞质,配对癌旁正常组织NF-E1b的表达阳性率为17.8%(8/45),明显低于癌组织的67.9%(114/168),差异有统计学意义(X^2=36.376,P=0.000)。NF-E1b在结直肠癌组织中的表达水平与患者的年龄(X^2=4.862,P=0.030)、TNM分期(X^2=10.969,P=0.002)、淋巴结转移(X^2=7.390,P=0.008)以及肿瘤远处转移(X^2=17.887,P=0.000)有关。中位随访时间23(1~77)月;NF-E1b高表达患者的5年总体生存率为18.4%,低表达者为56.6%,差异有统计学意义(P=0.000)。单因素分析显示,肿瘤位置(P=0.034)、肿瘤大小(P=0.003)、TNM分期(P=0.000)、肿瘤浸润深度(P=0.009)、淋巴结转移(P=0.000)、肿瘤远处转移(P=0.000)及NF—E1b表达水平(P=0.001)与患者预后有关。多因素预后分析显示,肿瘤〉4cm(HR=2.193,95%CI:1.334~3.603,P=0.002)、发生远处转移(HR=2.064,95%CI:1.160~3.672,P=0.014)及NF—E1b高表达(HR=1.994,95%CI:1.068~3.724,P=0.030)是影响结直肠癌患者总体生存的独立危险因素。结论NF-E1b在结直肠癌组织中表达水平较高.且与结直肠癌患者的不良预后有关。NF-E1b可能成为结直肠癌靶向治疗的新靶点。Objective To explore the expression of NF-Elb in colorectal cancer tissues and its association with various clinicopathological parameters and prognosis of the patients. Methods Clinicopathological and follow-up data of 168 colorectal cancer patients undergoing radical operation at Department of Gastrointestinal Surgery, Peking University Cancer Hospital & Institute from 2005 to 2012 were retrospectively analyzed, including 96 males and 72 females, with mean age of (57.8 ± 11.2) years. The expression of NF-Elb protein was detected in samples of 168 resected colorectal cancer tissues and 45 adjacent non-cancerous tissues by immunohistochemistry. The expression rates of NF-Elb were compared among different clinicopathological features. Moreover, the association between NF-Elb expression and prognosis was analyzed. Results The expression of NF-Elb protein located mainly in cytoplasm. Positive rate of NF-Elb expression in adjacent non-cancerous tissues was 17.8% (8/45), which was obviously lower than 67.9% (114/168) of cancer tissues with significant difference (X^2= 36.376, P= 0.000). Clinicopathological parameters analysis suggested that the expression level of NF-E1b in cancer tissues was associated with age (X^2= 4.862, P= 0.030), TNM staging (X^2 = 10.969, P= 0.002), lymph node metastasis (X2= 7.390, P=0.008) and distal metastasis (X^2= 17.887, P= 0.000). The median follow-up time was 23 (1-77) months. The overall 5-year survival of this cohort was 33.3%.
