摘　　要：目的:探讨胃泌素释放肽前体(progastrin releasing peptide,Pro GRP)在肺癌患者中的表达及临床意义。方法:采用电化学发光免疫法检测37例住院小细胞肺癌及76例非小细胞肺癌初治患者治疗前和同期62例本院健康体检者的血清Pro GRP、神经元烯醇化酶(neuron-specific enolase,NSE)的浓度,并分析小细胞肺癌的临床特征资料。结果:小细胞肺癌组Pro GRP浓度明显高于非小细胞肺癌组及健康对照组(F=10.730,P<0.001);Pro GRP诊断小细胞肺癌的灵敏度(67.6%)高于NSE(56.8%),二者联合检测灵敏度达70.3%;Pro GRP的表达在小细胞肺癌中与NSE的表达阳性率(P=0.012)及是否有远处转移(P=0.027)有关,与性别、年龄、胸水、淋巴结数、吸烟无明显相关性。结论:Pro GRP是检出小细胞肺癌患者较灵敏的肿瘤指标,与NSE联合检测可提高小细胞肺癌检出率;Pro GRP在小细胞肺癌中表达与NSE的阳性率明显相关,且可能与小细胞肺癌的进展有关。Objective To investigate the expression of serum progastrin releasing peptide （ProGRP）and its clinical significance in patients with small cell lung cancer （SCLC）. Methods Western blot enhanced chemiluminescence was used to detect serum ProGRP and NSE concentration of 37 cases of SCLC, 76 cases of NSCLC and 62 healthy controls. The correlation between the expression of ProGRP and clinicopathologic features of small cell lung cancer was analyzed. The methods to analyze the data were ANOVA and Fisher exact probability. Results Serum ProGRP levels of SCLC group was higher than that in control group （F=10.730, P〈0.001 ）; The sensitivity of ProGRP （67.6%）was the highest when compared with NSE （56.8%）and the detection sensitivity （70.3%）was significantly improved as ProGRP combined with NSE; Overexpression of ProGRP in small cell lung cancer was significantly correlated with NSE positive expression rate （P=0.012）and distant metastasis（P=0.027） but without any association with other clinicopathologic features. Conclusions ProGRP has a certain clinical significance in detection and the detection rate of SCLC can be improved when combined ProGRP with NSE. The expression of ProGRP in SCLC is significantly correlated with the positive rate of NSE, and it may be related to the development of SCLC.
