摘  要:目的:研究鼻咽癌放疗抵抗相关microRNA差异表达谱的建立。方法:选取10例初次治疗鼻咽癌的患者作为研究对象,将所有研究对象的放疗前的血浆标本,收集患者放疗前以及放疗3个月后鼻咽部MR图像并量化鼻咽癌的放疗敏感性,并测定放疗敏感及抗拒患者血浆中microRNA差异。探讨鼻咽癌患者血浆miRNA和放疗敏感性的关系,筛选能够预测放疗敏感性的血浆microRNA标记物。结果:通过比较发现差异表达microRNA192个,在这192个microRNA中,上调有107个,下调有85个,差异具有统计学意义(P<0.05),其中明显差异的microRNA有50个,Fold-Change>1.00(P<0.05)。14个下调的microRNA所对应的靶基因富集在4条信号传导通路:肿瘤相关通路,嘌呤代谢途径,Wnt信号通路以及轴突导向通路,差异有统计学意义(P<0.05);上调的microRNA没有较为显著富集的传导通路。结论:通过对放疗敏感和抗拒鼻咽癌患者血浆中miRNA的表达谱进行比较研究,经过筛选、验证,确定放疗敏感相关差异血浆miRNA,为鼻咽癌的临床个体化治疗提供依据。Objective To establish the microRNA expression profiles related of radiotherapy resistance in nasopharyngeal carcinoma. Method 10 cases of the primary treatment of nasopharyngeal carcinoma patients were chosen in our research,the plasma samples of all the subjects of the study before radiotherapy,patients were collected before radiotherapy and radiotherapy after 3months nasopharyngeal MR image,the quantify the radiation sensitivity of nasopharyngeal carcinoma,difference of radiotherapy sensitivity and resistance and determination of microRNA in plasma of patients. To investigate the relationship between plasma miRNA and radiosensitivity in nasopharyngeal carcinoma patients,screening of plasma microRNA markers able to predict the radiosensitivity. Results Compare the selection of differentially expressed and microRNA 192,in which raised 107,down 85,the difference was significant( P〈0. 05),the significant difference of microRNA 50,Fold- Change 1. 00( P〈0. 05). Target genes corresponding to 14 down regulated microRNA were enriched in 4 pathways: tumor associated through,purine metabolic pathway,Wnt signaling pathway and axon guidance pathway( P〈0. 05); The upregulation of microRNA had no more significant enrichment pathway. Conclusion Through comparative study on the expression of radiotherapy sensitivity and resistance of miRNA in plasma of patients with nasopharyngeal carcinoma in the spectrum,after screening,validation,determine the radiosensitivity related differences in plasma miRNA,provide the basis for clinical individual therapy of nasopharyngeal carcinoma.
