高危型人乳头瘤病毒E_6/E_7 mRNA检测对宫颈高度鳞状上皮内病变诊断及随访价值研究

摘　　要：目的探讨高危型人乳头瘤病毒(HPV)E_6/E_7 mRNA检测对宫颈高度鳞状上皮内病变(HSIL)+的诊断价值及HSIL-的随访意义。方法收集2010年1月至2010年12月在郑州大学第二附属医院常规行宫颈癌筛查的妇女共2527例,均行液基细胞学检查(TCT)+高危型HPV E_6/E_7 mRNA检测。任一结果为阳性均行阴道镜活检,病理结果>HSIL为病理阳性,≤HSIL为病理阴性。结合年龄、TCT、活检病理,分析高危型HPV E_6/E_7 mRNA检测诊断HSIL+的价值;对活检病理阴性患者随访5年,分析该检测对HSIL-的随访价值。结果高危型HPV E_6/E_7 mRNA检测阳性率23.39%(591/2527);E_6/E_7 mRNA表达量与宫颈病变分级呈正相关(P<0.01);对病理阴性患者随访,HPV E_6/E_7 mRNA阳性患者较阴性患者罹患HSIL+的风险增加(P<0.05)。结论高危型HPV E_6/E_7 mRNA检测在宫颈癌筛查中对HSIL+的诊断及HSIL-的随访均有较高临床价值。Objective To explore the clinical significance of high-risk HPV EriE7 mRNA test in screening of HSIL＋ and follow-up of HSIL- Methods Betwden January 2010 and December 2010,2527 outpatients in the Second Affiliat- ed Hospital of Zhengzhou University received QuantiVirus cytology test and high-risk HPV EJE7 mRNA test voluntari- ly for cervical cancer screening.Patients with either positive results were referred to colposcopy.Pathological diagnosis 〉 HSIL was considered positive, while 〈HSIL negative.Combining age,TCT and pathological biopsy, the value of high-risk HPV EjE7 mRNA test in diagnosis of HSIL＋ was analyzed. Patients with negative biopsy were referred to follow-up visit of 5 yesrs to analyze the value of this test for HsIL-. Results Positive rate of HPV E6/E7 mRNA test was 23.39% （591/ 2527）.A positive correlation was found between the expression of EJE7 mRNA and grade of cervical lesion（P 〈 0.01 ）.In the follow-up of patients with negative biopsy result, those with positive HPV results had a higher chance of developing HSIL＋than negative ones （P 〈 0.05）.Conclusion High-risk HPV EJE7 mRNA test has high clinical value in diagnosis of HSIL＋ and follow-up of HSIL- in screening of cervical cancer.
