HPV检测在宫颈癌筛查和治疗中的临床意义

摘　　要：目的研究人乳头状瘤病毒（HPV）检测在宫颈癌筛查和治疗中的临床意义。方法选取548例行宫颈癌筛查的女性，先使用液基薄层细胞检测（TCT）进行检查的程序及结果表示为TCT检查，TCT检查后再使用HPV进行检查，过程及结果称之为HPV筛查，检查结果参照病理结果进行分析。结果HPV和TCT阳性情况均与实际结果相同；HPV检查对良性炎症及宫颈上皮内瘤变1、2、3级的阳性率都远高于TCT检查的阳性检出率，差异有统计学意义（P〈0．05）。HPV阳性与实际阳性率比较差异未见统计学意义（P〉0．05）；HPV阳性与TCT阳性相比有很大区别，差异有统计学意义（P〈0．05）；TCT阳性与实际阳性率比较差异有统计学意义（P〈0．05）。HPV阳性感染者中，有单一感染因素导致的疾病率高于多型感染导致的发病率，差异有统计学意义（P〈0．05）。结论HPV病毒与宫颈癌的严重程度有密切的相关性，对HPV进行筛查可以较准确的检测出宫颈癌的发生情况，对宫颈癌的治疗及预防具有非常重要的临床意义。Objective To study the clinical significance of HPV detection in cervical cancer screening and treatment. Methods A total of 548 women for cervical cancer screening were selected to examine the procedure and the results of TCT examination, and then use HPV to check the process and the result was called HPV screening, and the results were analyzed according to the pathological findings. Results The positive detection rate of HPV and TCT was 100%, and the positive rate of benign inflam- mation, CIN1, CIN2 and CIN3 examined by HPV was much higher than that examined by TCT, the difference was significant （ P 〈 0. 05 ）. The difference between the actual positive rate and the positive rate of HPV had no significant difference （P 〉 0.05 ）. The difference between the actual positive rate and the positive rate of TCT was significant （ P 〈 0. 05 ）. The incidence of single factor infection was higher than that of multiple factors infection, there was significant difference （P 〈 0. 05 ）. Conclusions HPV virus has a close correlation with the severity of cervical cancer, and it can be more accurate to screen HPV, which has a very important clinical significance for the treatment and prevention of cervical cancer.
