皂角粉吸鼻诱导咳嗽对肺癌根治术后肺部并发症的预防效果研究

摘　　要：目的比较皂角粉吸鼻诱导咳嗽与另外两种方法在预防开胸肺癌根治术后肺部并发症中的效果。方法选择2013年1月-2015年1月收住我院的120例拟行开胸肺癌根治术的患者,术前随机分成3组:皂角粉组(A组)、手法拍背组(B组)和机械排痰组(C组),将临床资料进行对比研究。结果 3组患者在术后引流管留置时间、引流量、术后住院天数、术后发生肺不张、需要行纤支镜吸痰、需再次穿刺或置管引流方面比较,差异均有统计学意义(P<0.05)。且在上述方面,A组均要优于B组,差异均有统计学意义(P<0.05)。A组与C组术后胸腔引流量分别为(614.05±112.01)mL和(639.73±141.23)mL;发生肺不张例数分别为4例和3例;以及由此需要行纤支镜吸痰的各为2例,差异无统计学意义(P>0.05)。而术后引流管留置时间(3.92±1.09)d vs(4.76±1.34)d、术后住院天数(9.30±1.60)d vs(10.14±1.70)d,A组要短于C组,差异有统计学意义(P<0.05)。3组患者在术后24h、48h、72hVAS疼痛评分比较差异无统计学意义(P>0.05)。结论使用皂角粉吸鼻诱导患者咳嗽排痰法能够有效减少开胸术后肺部并发症的发生率。减少引流管留置时间及术后住院天数,而且皂角粉因其价格便宜,容易采购,使用方法简单,无需耗资购进专用设备,适合在基层医院开展应用。Objective To compare the effect of cough induced by saponin powder nasal suction with the other two ways in the protection of pulmonary complications after radical resection of lung cancer.Methods 112 patients with lung cancer undergoing radical operation for lung cancer was enrolled from January 2013 to January 2015,and were randomly divided into three group,Saponin powder group（group A）,manual backslapping group（group B）and Mechanical sputum group（group C）.Clinical data for the three group was comparative study.Results There were statistically significant differences among 3groups with the time of extracting drainage tube,volume of drainage,postoperative hospital stay,incidence of complication and thoracocentesis（P0.05）.Group A was better than group B compared to in these mentioned aspects（P 0.05）.Postoperative thoracic drainage for group A and group C were（614.05±112.01）mL and（639.73±141.23）mL;The number of pulmonary cases occurred were 4cases and 3cases for group A and group C;The need to do a bronchoscopy suction was 2cases for each group.There was no significant difference（P 0.05）.The postoperative drainage tube indwelling time was（3.92±1.09）d vs（1.34±4.76）d,postoperative hospital days was（9.30±1.60）d vs（10.14±1.70）d for group A and group C,There was significant difference cant（P0.05）.There was no significant difference in 24 h,48h,VAS 72 hpain scores among the 3groups（P0.05）.Conclusion Using saponin powder can effectively reduce the incidence of pulmonary complications after thoracotomy,decrease drainage tube indwelling time and postoperative hospital stay.Furthermore,saponin powder is suitable for using in primary hospital because of its cheap,easy to purchase,simple to use and without the expense of purchasing special equipment.
