16层螺旋CT多期增强扫描对小肝癌的诊断研究

摘　　要：目的:研究分析16层螺旋CT多期增强扫描对小肝癌的诊断价值。材料与方法:随机选取医院收治的2008年1月到2010年12月53例经手术病理证实的小肝癌患者,对53例小肝癌患者临床资料进行回顾性分析,并且所有小肝癌患者均16层螺旋CT多期增强扫描诊断检查。结果:通过16层螺旋CT多期增强扫描共诊断出其中43例伴有肝硬化,检出小肝癌病灶70个;16层螺旋CT多期增强扫描对小肝癌患者在动脉期、门静脉期、延迟期三期扫描诊断率分别为97.1%(68/70)、82.9%(58/70)、87.1%(61/70)。对比手术病理诊断,16层螺旋CT多期增强扫描诊断与之对比,三期综合检出率增高至100%。临床中,运用16层螺旋CT多期增强扫描诊断小肝癌,其小肝癌的三期扫描表现以及特点为,在动脉期呈现为高密度灶,在门脉期则为低密度灶,延迟期呈低密度,分别占91.4%(64/70)、74.3%(52/70)、87.1%(61/70)。结论:在临床小肝癌诊断中,运用16层螺旋CT多期增强扫描,还可以清晰反映小肝癌的血供变化特点,为临床早期诊断小肝癌提供可靠的影像学依据,在小肝癌早期诊断、治疗以及病情监测方面,发挥重要作用。Purpose： To study the diagnostic value of 16 slice spiral CT in the diagnosis of small hepatocellular carci- noma. Materials and Methods： the clinical data of 53 patients with small HCC confirmed by surgery and pathology were ran- domly selected from December 2010 to January 2008, and the clinical data of 53 cases of small liver cancer patients were retro- spectively a_,＇.atyzed, and all patients with small hepatocellular carcinoma were 16 slice spiral CT multi phase enhanced scan di- agnostic tests. Results： 43 cases with liver cirrhosis, 70 cases with liver cirrhosis were diagnosed by the 16 slice spiral CT, and 16 patients with small hepatocellular carcinoma were detected with slice spiral CT scanning. The diagnostic rate of the three stage was 97.1% （68/70）, 82. 9% （58/70）, 87. 1% （61/70）, and the three phase was 100%. The 16 phase en- hanced scan of the slice spiral CT in the three phase of small liver cancer was characterized by high density of arterial phase, low density, low density, 91.4% （64/70）, 87.1% （52/70）, 74. 3% （61/70）, respectively. Conclusion： in the clinical diagnosis of small liver cancer, the use of 16 slice spiral CT multi phase enhanced scan, it can clearly reflect the characteris- tics of small liver cancer for early diagnosis of small liver cancer to provide a reliable basis for early diagnosis, treatment and disease monitoring, play an important role in the early diagnosis of small liver cancer.
