胃癌患者血清中性粒细胞明胶酶相关载脂蛋白水平及临床意义

摘　　要：目的探讨胃癌患者外周血中中性粒细胞明胶酶相关载脂蛋白(neutrophil gelatinase-associated lipocalin,NGAL)与其侵袭、转移及预后的关系。方法收集深圳市龙岗区人民医院2011年1月-2012年6月40例胃癌患者和28名健康体检者的血清标本,通过酶联免疫吸附法(ELISA)检测血清NGAL水平。分析血清NGAL水平与胃癌临床病理参数的关系;通过受试者ROC曲线分析确定血清NGAL与癌胚抗原(CEA)诊断胃癌的诊断性能。通过多因素分析,明确血清NGAL水平与胃癌患者预后的关系。结果胃癌患者血清NGAL水平明显高于健康对照者。血清NGAL水平与Lauren’s分型、淋巴结转移、浸润深度、远处转移及TNM分期有关(P<0.05)。NGAL取12.51 ng/ml作为血清诊断胃癌的最佳截点值,其灵敏度为84%,特异度为90%。NGAL阳性和阴性患者生存期中位数分别为12个月和27个月,差异有统计学意义(P<0.05)。结论血清NGAL水平与胃癌的发生、浸润和转移相关,用于胃癌早期筛查、预后评估有重要的临床应用价值。Objective To explore the clinical significance of serum neutrophil gelatinase-associated lipocalin（ NGAL） levels in human gastric cancer. Methods Serum NGAL level was determined by enzyme-linked immunosorbent assay（ ELISA） in 40 gastric cancer patients and 28 healthy controls who were collected from the Longgang District People＇s Hospital of Shenzhen from Jan. 2011 to Jun. 2012. The relationship of serum NGAL level and clinical pathological features in gastric cancer patients was examined,and then its diagnostic performance for screening gastric cancer was evaluated by ROC curve between NGAL and CEA. Otherwise,Cox survival analysis was performed to examine the relationship between serum NGAL level and prognosis in human gastric cancer. Results Serum NGAL level in patients with human gastric cancer was significantly higher than that in healthy controls（ P〈 0. 01）,and it was correlated with Lauren＇s classification,lymph node metastasis,vascular invasion,distant metastasis and the TNM stage of gastric cancer. Furthermore,when the optimal cut-off point of serum NGAL level for diagnosing gastric cancer was 12. 51 ng / ml,the sensitivity and specificity were 84% and 90%. Conclusion The potential of serum NGAL level as an early marker and its ability to predict the prognosis of patients with gastric cancer are estimated.
