摘  要:目的 通过荟萃分析对异常肌反应(AMR)监测在面肌痉挛(HFS)微血管减压手术(MVD)中的作用进行评价.方法 回顾性分析在我科接受治疗的33例典型面肌痉挛患者临床资料,计算机检索数据库中关于AMR监测与面肌痉挛MVD术后疗效关系的临床研究,采用RevMan5.3荟萃分析软件进行分析,采用优势比(OR)对术中AMR变化程度与术后疗效的关系进行评价.结果 AMR消失组,显著变化组,未消失组的手术有效率分别为96.6%,50%,50%,组间差异具有统计学意义(P<0.05);荟萃分析显示AMR消失组的术后治愈率是AMR未消失组的2.94倍[OR=2.94,95% CI为(2.03~4.27)].结论 在面肌痉挛微血管减压术中进行AMR监测可有效提高手术治愈率,应将AMR作为面肌痉挛MVD术中常规电生理监测项目.Objective The value of abnormal muscle response (AMR) monitoring during microvascular decompression surgery (MVD) for bemifacial spasm (HYS) through a meta-analysis was discussed. Methods A total of 33 HFS patients with AMR monitoring during MVD were studied retrospectively and the literatures about AMR monitoring during MVD were identified from the relevant databases. The statistical analysis was conducted by the software of RevMan 5.3. The relationship between the change of AMR during MVD and the clinical outcomes after surgery was evaluated by odds ratio (OR). Results The effective rate of surgery was 96. 6%, 50%, and 50% in AMR cotrrplete-disappeared group, partial-disappeared group and non-disappeared group, respectively, with significant statistical difference. Meta-analysis revealed that the cure rate of HFS in AMR disappeared group was 2.94 times greater than that of AMR-non-disappeared group [ OR =2.94; 95% CI(2.03 -4.27) ]. Condusion AMR monitoring during MVD can be very useful to improve the outcomes after surgery, so it should be popularized in clinical practices.
