RFA联合PEI及门静脉灌注治疗门静脉癌栓的疗效分析

摘　　要：目的探讨超声引导下经皮射频消融联合无水酒精硬化及门静脉灌注治疗门静脉癌栓(PVTT)的安全性、有效性。方法 41例原发性肝癌合并PVTT患者,分成两组治疗,观察组21例采用经皮射频消融联合无水酒精硬化治疗及门静脉灌注治疗;对照组20例采用单纯经皮射频消融治疗。比较两组PVTT患者的生存率及复发率,分析影响生存率的相关因素。结果 41例均顺利完成PVTT治疗,发生肝脓肿1例。观察组治疗后6、12、24、36个月的生存率明显高于对照组,复发率明显低于对照组,差异有统计学意义(P<0.05)。结论超声引导下经皮射频消融联合无水酒精硬化治疗及门静脉灌注化疗治疗PVTT疗效明显,可延长患者的生存期,对PVTT的形成有明显的治疗和预防作用。Objective To explore the safety and efficacy of ultrasound guided percutaneous radiofrequency ablation combined with percutaneous alcohol sclerotherapy and portal vein perfusion of PVTT treatment. Methods A total of 41 cases with primary HCC and PVTT were divided into two groups,observation group（ 21 cases） were treated by percutaneous radiofrequency ablation combined with percutaneous alcohol sclerotherapy and portal vein perfusion treatment; control group（ 20 cases） were treated with simple percutaneous radiofrequency ablation. The survival rate and recurrence rate were compared between the two groups,and the related factors affecting the survival rate were analyzed. Result PVTT treatment was successfully completed in 41 cases,1 cases with liver abscess. The 6,12,24,36 months survival rate of the observation group was significantly higher than that of the control group,the recurrence rate was significantly lower than that of the control group,the difference was statistically significant（ P 0. 05）.Conclusion Ultrasound guided percutaneous radiofrequency ablation combined with percutaneous alcohol sclerotherapy and portal venous infusion chemotherapy for the treatment of PVTT obvious curative effect,can prolong the survival time of the patients and of PVTT formation has obvious treatment and prevention.
