摘 要:目的探讨超声与X线平片联合诊断膝关节滑膜骨软骨瘤病(SO)的临床应用价值。方法以手术和病理结果作为金标准,比较51例膝关节SO患者的术前超声、X线平片及X线平片+超声三种方法对SO结节及继发性骨关节病的显示情况。结果超声对软骨结节的显示率(93/98)明显高于X线平片(0/98),差异有统计学意义(χ^2=176.971,P〈0.05);超声对关节周围钙化结节的显示率(489/545)高于X线平片(403/545),差异有统计学意义(χ^2=45.645,P〈0.05);X线平片对关节间隙深部钙化结节的显示率(12/15)明显高于超声检查(0/15),差异有统计学意义(χ^2=20.000,P〈0.05);X线平片和超声联合检查SO结节的显示率(634/658)明显高于单一X线平片检查(415/658)单一超声检查(582/658),差异均有统计学意义(χ^2=225.350、29.264,均P〈0.05)。X线平片显示继发性骨关节病20个,超声显示了其中10个,差异有统计学意义(χ^2=4.640,P〈0.05)。结论超声和X线平片联合诊断能取长补短,明显提高结节显示率,具有很高的临床应用价值。Objective To explore the clinical value of the combination of ultrasound and X-ray in knee synovial osteochondromatosis(SO). Methods In 51 patients with knee SO proved by surgery and pathology,the differences of the three methods(ultrasound,X-ray,the combination of X-ray and ultrasound)in revealing the SO nodules and secondary bone and joint lesions were compared. Results The ultrasound identification rate(93/98)of cartilaginous nodules was higher than that of plain X-ray(0/98),there was significant difference(χ~2=176.971,P〈0.05). The ultrasound identification rate(489/545)of the calcific nodules around the joints was higher than that of plain X-ray(403/545),there was significant difference(χ~2=45.645,P〈0.05).The X-ray identification rate(12/15)of the calcific nodules deep under the joints was higher than that of ultrasound(0/15),there was significant difference(χ~2=20.000,P〈0.05). The identification rate(634/658)of combination diagnosis method for all the SO nodules was higher than that of X-ray(415/658)and ultrasound(582/658),there was significant difference(χ~2=225.350,29.264,P〈0.05). Twenty secondary bone and joint lesions were revealed by X-ray,only 10 by ultrasound,there was significant difference(χ~2=4.640,P〈0.05). Conclusion The combination of ultrasound and X-ray could overcome the shortcomings of the two methods.The nodule identification rate could be improved,and it has high clinical application value.
