抑制素A激活素A联合监测在先兆流产中的临床价值

摘　　要：目的探讨抑制素A、激活索A联合监测在先兆流产诊断及预后评估中的临床价值。方法137例孕妇分为正常妊娠孕妇45例（A组），有先兆流产症状但经过适当临床治疗继续妊娠的孕妇50例（B组），有先兆流产症状且最终发展成为难免流产的孕妇42例（C组）。检测各组孕妇在5、6、7、8孕周时血清激活素A及抑制素A水平变化，比较单一检测和联合检测的敏感性。结果C组5、6、7、8孕周血清激活素A、抑制素A水平明显低于A组和B组（P〈0．05），B组血清激活素A、抑制素A水平也明显低于A组（P〈0．05），且B、C两组血清激活素A、抑制素A水平随孕周增加而明显升高和降低（P〈0．05），A组不同孕周血清激活素A、抑制素A水平比较差异无统计学意义（P〉0．05）；血清激活素A及抑制素A联合检测难免流产的敏感性（90．5％）明显高于单一检测（78．6％，85．7％）（P〈0．05）。结论血清抑制素A及激活素A联合监测在先兆流产诊断及预后评估中具有较大的临床价值，值得推广应用。Objective To study the clinic value of the combined detection ofinhibin A and activin A in the diagnosis and evaluation of prognosis of threatened abortion. Methods 137 cases of pregnant women were divided into normal pregnant women （ A group, n=45 cases ） , with symptoms of threatened abortion, but after appropriate clinical treatment of pregnant women （B group, n=50 cases ） , has threatened abortion symptoms and eventually developed into the inevitable abortion of pregnant women （ C group, n=42 cases ） . The serum activin A and inhibin A levels of all groups were detected in 5, 6, 7, 8 gestational week, and the sensitivity of single and combined detection were compared. Results The serum activin A and inhibin A levels of C group were lower than those of A group and B group in 5, 6, 7, 8 gestational week （ P〈0.05 ） . The serum activin A and inhibin A levels of B group were significantly lower than that of A group （ P〈0.05 ） , and with gestational age increased, the serum activin A and inhibin A levels of B, C groups significantly increased and decreased （ P〈0.05 ） . The serum activin A and inhibin A levels of group A at different gestational weeks had no statistical significance differences （ P〉0.05 ） . The sensitivity of combined detection of serum activin A and inhibin A （ 90.5% ） was significantly higher than that of single detection （ 78.6%, 85.7% ） （ P〈0.05 ） . Conclusion The combined detection of serum inhibin A and activin A in the diagnosis and evaluation of prognosis of threatened abortion has a great clinical value, it is worthy of popularization and application.
