多曲方丝弓与摇椅弓对深覆治疗效果的对比研究

摘　　要：目的 应用PAR指数结合头影测量的方法对多曲方丝弓与摇椅弓治疗深覆的临床效果进行评估比较。方法 选取深覆Ⅱ-Ⅲ度青少年患者30例,均为均角型,多曲方丝弓组和摇椅弓组各15例,所有病例采用直丝弓矫治技术,对比治疗前、后的PAR指数和头影测量值,进行统计学分析,评估两组正畸治疗效果。结果 两组病例矫治后各项PAR分值比治疗前明显减小,差异有统计学意义（P〈0.05）。其中,多曲方丝弓组中的颊侧咬合关系及覆的PAR分值减少量大于摇椅弓组（P〈0.05）。权重PAR总分差大于摇椅组（P〈0.05）。多曲方丝弓组对上下切牙的压低效果比摇椅弓组明显,且对下切牙的唇倾度控制的较摇椅弓组好（P〈0.05）。多曲方丝弓组治疗前后对后牙的升高及下颌平面角、下面高和全面高各测量值没有摇椅弓组升高的明显（P〈0.05）。结论 两组矫治方法对深覆的治疗效果都有明显的改善,但多曲方丝弓组比摇椅组改善的更明显,尤其是覆及颊侧咬合关系两方面改善更明显。而摇椅弓组对颌骨和面型有影响。Objective To compare the clinical effects of the multiloop edgewise arch wire （MEAW） and reverse- curve arch in the treatment of anterior deep overbite. Methods Thirty adolescents patients with deep overbite were treated with MEAW or reverse-curve arch. All the cases were trated with straight wire arch. Cephalogram and casts were taken and measured before and after treatment. Results The PAR score of the two groups significantly decreased than before treatment （P 〈 0.05）. The reduced weight of the buccal occlusion and overbite PAR score in the MEAW group was greater than that in the reverse-curve arch group （ P 〈 0.05 ）. The total PAR score of MEAW group was greater than that of reversecurve arch group （P 〈 0.05 ）. The intrusion effect of upper and lower incisors of MEAW group was more significant than that of reverse-curve arch group, and MEAW had better control of the lip inclination of teeth than reverse-curve arch （P 〈 0.05）. The measure values of the rise of posterior teeth , MPA, ANS-Me and N-Me in reverse-curve arch group increased more significantly than in the MEAW group （P 〈 0.05 ）. Conclusion The treatmeant outcome of the overbite by the two methods all greatly improved, iBut MEAW proup improved more sinificantly than the reverse-curve arch group.
