克罗恩病患者血清维生素D水平与疾病活动性的相关性研究

摘　　要：目的探讨克罗恩病(Crohn's disease,CD)患者血清维生素D水平与疾病活动性及治疗的关系。方法采用25-羟基维生素D酶联免疫试剂盒检测19例活动期CD患者和16例缓解期CD患者血清维生素D水平并进行比较,运用Spearman秩相关系数判断血清维生素D与疾病活动性评分严重程度评分(CDAI)之间的关系;比较6例活动期CD患者在英夫利西单抗治疗前后的维生素D水平和CDAI评分。结果活动期CD患者的维生素D水平显著低于缓解期患者[(23±5)ng/ml vs(31±5)ng/ml,P<0.001];所有患者的血清维生素D水平与CDAI评分呈负相关(Spearman's r=-0.6,P<0.001);英夫利西单抗治疗后维生素D水平显著高于治疗前[(39±10)ng/ml vs(24±6)ng/ml,P<0.01]。治疗后CDAI显著低于治疗前(242±73 vs 363±115,P<0.01)。结论血清维生素D水平与克罗恩病的疾病活动性负相关,英夫利西单抗治疗后维生素D水平升高提示维生素D水平升高可以作为CD治疗效果的一项评价指标。Objective To explore the relationship between serum vitamin D levels and disease activities as well as its treatment in Crohn＇s disease （CD） patients. Methods Serum vitamin D levels of the 19 patients with active CD and 16 patients in clinical remission were detected by 25（OH）-vitamin D enzyme-linked immunoassay kit. The correlation between serum vitamin D levels and disease activities was analyzed by Spearman＇s r. The vitamin D levels and CDAI scores of 6 active CD patients were compared before and after infliximab treatment. Results Patients with active CD had lower serum vitamin D levels than those in clinical remission （23±5 ng/ml vs 31 ±5 ng/ml,P〈0.001）. Serum vitamin D levels from all the patients were correlated with CDAI scores （Spearman＇s r=-0.6,P〈0.001）. Vitamin D levels were 24 ±6 ng/ml before infliximab treatment, while they were 39 ±10 ng/ml after treatment,with significant increases （P〈0.01）. Moreover,significant decreases in CDAI scores were noted in patients （242 ±73 vs 363 ±115,P〈0.01）. Conclusion Low vitamin D levels are associated with disease activity in CD and increase after infliximab treatment.
