C反应蛋白动态变化与食管静脉曲张套扎术后再出血的相关性研究

摘　　要：目的分析食管静脉曲张套扎术后,C反应蛋白的动态变化与患者术后再出血的关系。方法选择研究对象为2009年6月至2015年6月我院收治的肝硬化并发食管静脉曲张患者130例,均行胃镜下食管静脉曲张套扎术(EVL)治疗,在手术后6、24及48h行血清C反应蛋白监测,分析C反应蛋白动态变化与术后患者再出血之间的关系。结果经过积极抢救后,EVL术后再出血患者有15例,再出血率为11.5%,其中有6例患者死亡,再出血病死率为40%;在术后6、24、48h时,研究组患者的C反应蛋白水平均明显高于对照组,组间差异有统计学意义(P<0.05)。结论患者EVL术后具有较高的再出血风险,应加强对患者的病情监测。Objective To analyze the relationship of dynamic changes of the serum C-reactive protein with the rehaemorrhagia after the esophageal varices band ligation.Methods The research subjects of the study was130 patients with liver cirrhosis complicated with esophageal varices in our hospital from June 2009 to June 2015,and these 130 cases were all underwent endoscopic variceal ligation（EVL）treatment.6h,24 hand 48hafter the surgery,monitoring the serum C-reactive protein,and analyzing the relationship between C-reactive protein dynamics and postoperative bleeding was performed.Results After a positive rescue,there were 15 cases of postoperative bleeding after EVL,the rebleeding rate was 11.5%including 6patients died.The rebleeding mortality rate was 40%;at the 6h,24 hand 48hafter surgery,the average C-reactive protein levels in research group were significantly higher than that in control group,the difference was statistically significant（P〈0.05）.ConclusionAfter EVL surgery,patients have a high risk of rebleeding.The monitoring of patients＇ condition should be reinforced and eating time should be postponed.
