摘 要:目的观察细胞周期蛋白(Cyclin)家族成员在三阴型乳腺癌(triple-negative breast cancer,TNBC)和激素依赖型乳腺癌中的表达情况,筛选差异表达Cyclin,探讨其与TNBC临床病理参数的关系及对预后判断的指导意义。方法收集2008年1月—2014年5月沈阳军区总医院乳腺癌手术切除组织,选出65例TNBC组织和45例激素依赖型乳腺癌组织,应用实时荧光定量PCR检测Cyclin的基因表达,分析Cyclin在两种类型乳腺癌中的差异表达,筛选出特异性表达的Cyclin成员,并在乳腺癌组织样本中进行免疫组织化学染色检测及结果验证。应用统计学方法分析Cyclin表达与TNBC患者临床和病理资料及其与总体生存期之间的关系。结果 Cyclin G1和Cyclin D1的mRNA以及蛋白在TNBC组织中的表达高于激素依赖型乳腺癌组织(P<0.05,P<0.01)。临床分析显示Cyclin G1和Cyclin D1蛋白表达均与TNBC患者的绝经情况、淋巴结转移、P53表达和复发转移有相关性(P<0.05)。Cyclin G1和Cyclin D1蛋白高表达的TNBC患者其总体生存期均小于低表达的TNBC患者(P<0.05)。结论 Cyclin G1和Cyclin D1基因可能参与TNBC的发生和发展;Cyclin G1和Cyclin D1蛋白的表达可能成为TNBC预后预测的指标。Objective To observe differential expressions of cyclin families in triple negative breast cancer( TNBC) and hormone-dependent breast cancer( HDBC),and to choose cyclin differential expression to study its relationships with clinicopathological parameters and the prognostic significance in different types of breast cancer. Methods Breast cancer tissues by surgical ablation during January 2008 and May 2014 were collected,and 65 TNBC and 45 HDBC pathological samples were chosen in this study. Real-time fluorescent quantitation polymerase chain reaction( PCR) was used to detect cyclin expressions,and its differential expressions in the two types of breast cancer were analyzed. The specific expressions of cyclin members were chosen,and then were detected using immunohistochemical staining and validated the results. The relationships between cyclin expressions with clinicopathological data and overall survival time of TNBC patients were analyzed using statistical method. Results Expressions of cyclin G1 and cyclin D1 mRNA and protein were higher in TNBC tissues than those in HDBC tissues( P〈0. 05,P〈0. 01). The clinical analysis showed that cyclin G1 and cyclin D1 protein expressions correlated with menopausal status,lymph node metastasis,P53 expression,metastasis and recurrence in TNBC patients( P〈0. 05). The overall survival time of high expressions of cyclin G1 and cyclin D1 protein were shorter than that of low expressions in TNBC patients( P〈0. 05). Conclusion Cyclin G1 and cyclin D1 may be involved in incidence and development of TNBC and protein expressions of cyclin G1 and cyclin D1 may be the prognostic indicators of TNBC.
