摘 要:目的探讨乙醛脱氢酶1(ALDH1)在肿瘤细胞质中表达和在肿瘤细胞基质中表达与乳腺癌患者预后的关系。方法选取该院乳腺外科接受手术的乳腺癌患者为研究对象,应用免疫组织化学方法检测160例乳腺癌患者的病理标本,研究ALDH1在肿瘤细胞质中表达和肿瘤基质中表达与患者年龄、月经史、肿瘤大小、TNM分期、淋巴结转移数目、ER、PR、HER-2、Ki-67等指标及临床预后的关系。结果 ALDH1在肿瘤细胞质中表达56例,占35%,在肿瘤细胞基质中表达64例,占40%。ALDH1在肿瘤基质中表达与患者年龄>50岁及ER表达情况有统计学意义(P=0.013;P=0.022)。在乳腺癌细胞质中ALDH1阳性表达较ALDH1阴性表达患者的无病生存期(RFS)和总生存期(OS)明显缩短,差异有统计学意义(P<0.05)。在乳腺癌基质中ALDH1阳性与乳腺癌患者预后生存时间差异均无统计学意义(均P>0.05)。结论在乳腺癌细胞质中ALDH1阳性表达较ALDH1阴性表达患者的RFS和OS明显缩短;在乳腺癌基质中ALDH1阳性表达与乳腺癌患者年龄及ER表达情况具有相关性,与其他临床参数无相关性;ALDH1在肿瘤基质中阳性表达对评价乳腺癌患者预后生存没有价值。Objective To explore the relationships between expressions of acetaldehyde dehydrogenase 1 (ALDH1) in cytoplasm and stroma of breast cancer cells and prognosis of breast cancer patients. Methods The pathological specimens of 160 breast cancer patients un- dergoing surgery in Department of Breast Surgery in the hospital were detected by immunohistochemical method. The relationships between expressions of ALDH1 in cytoplasm and stroma of breast cancer cells and age, menstrual history, size of tumors, TNM staging, the number of lymph node metastasis, estrogen receptor (ER) , progestogen receptor (PR) , human epidermalgrowth factor receptor-2 (HER-2) , Ki- 67 and clinical prognosis were studied. Results ALDH1 expressed in cytoplasm of breast cancer cells in 56 patients (35%) and stroma of breast cancer cells in 64 patients (40%) . There was statistically significant difference in the expression of ALDH1 in stroma of breast canc- er cells between the patients more than 50 years old and the patients aged or less than 50 years old (P=0. 013 ) ; there was statistically sig- nificant difference in the expression of ALDHI in stroma of breast cancer cells between the patients with positive ER and the patients with negative ER (P = 0. 022 ) . In cytoplasm of breast cancer cells, disease free survival (RFS) and overall survival (OS) of patients with posi- tive ALDH1 were statistically significantly shorter than those of patients with negative ALDH1 ( P〈0. 05 ) . In stroma of breast cancer cells, positive ALDH1 was not correlated with survival time of breast cancer patients 'after surgery ( P〉 0. 05 ) . Conclusion In cytoplasm of breast cancer cells, RFS and OS of patients with positive ALDH1 are significantly shorter than those of patients with negative ALDH1 ; in stroma of breast cancer cells, positive ALDH1 is correlated with age and ER expression, but it is not correlated with other clinical parameters; positive expression of ALDH1 in stroma of breast cancer cells has n
