乌灵胶囊联合重复经颅磁刺激治疗老年抑郁症的效果及对认知功能的影响

摘　　要：目的比较乌灵胶囊联合重复经颅磁刺激与单用重复经颅磁刺激对老年抑郁症的疗效及对认知功能的影响。方法收集2014年10月-2016年1月在广州医科大学附属第二医院精神科门诊就诊的120例抑郁发作患者,所有患者均符合精神疾病诊断与统计手册（DSM-Ⅳ）诊断标准。采用随机数字表法将其随机分成实验组和对照组,各60例。实验组采用乌灵胶囊联合重复经颅磁刺激治疗,对照组单用重复经颅磁刺激治疗,疗程8周。观察两组临床疗效,并于治疗前及治疗8周后分别行汉密尔顿抑郁量表（HAMD）及威斯康星卡片分类测验（WCST）评定。结果治疗8周后,实验组总有效率为88.33%,对照组总有效率为81.67%,两组总有效率比较差异无统计学意义（P〉0.05）。两组治疗8周后HAMD评分均较治疗前显著降低（P〈0.05）,但两组治疗8周后差异无统计学意义（P〉0.05）。实验组治疗8周后WCST测试中总应答数、持续错误数、完成分类数与治疗前比较,差异有统计学意义（P〈0.05）;对照组治疗8周后总应答数、完成分类数与治疗前比较,差异有统计学意义（P〈0.05）。两组治疗8周后WCST测试结果比较差异均无统计学意义（P〉0.05）。结论乌灵胶囊联合重复经颅磁刺激治疗老年抑郁症效果确切,且对认知功能具有一定的改善作用。Objective To compare the efficacy of Wuling Capsules combined with repetitive transcranial magnetic stimulation and single repetitive transcranial magnetic stimulation for senile depression and the influence on cognitive function. Methods One hundred and twenty cases of patients with paralepsy treated in the Outpatient Clinic of Psychiatry of the Second Affiliated Hospital of Guangzhou Medical University from October 2014 to January 2016 were collected, all patients were matched up with the diagnostic criteria of the diagnostic and statistical manual of mental disorders Ⅳ（DSM-Ⅳ）. According to the random number table, the patients were randomly divided into experimental group and control group, with 60 cases in each group. The experimental group was taken Wuling Capsules combined with repetitive transcranial magnetic stimulation, the control group was taken single repetitive transcranial magnetic stimulation, the course was 8 weeks. The clinical efficacy of the two groups was observed, the Hamilton depression scale（HAMD） and Wisconsin card sorting test（WCST） before treatment and after treatment for 8 weeks were evaluated. Results After treatment for 8 weeks, the total effective rate of experimental group was 88.33%, which of control group was81.67%, there was no significant difference of the total effective rate between the two groups（P〉0.05）. The HAMD scores of the two groups after treatment for 8 weeks were all significantly lower than those before treatment（P〈0.05）,while there was no statistically significant difference of the HAMD scores after treatment for 8 weeks between the two groups（P〉0.05）.
