急性自发性脑出血患者血清C反应蛋白及白细胞介素-6的变化及临床意义

摘　　要：目的探讨急性自发性脑出血患者血清C反应蛋白（CRP）及白细胞介素-6（IL-6）的变化及临床意义。方法选取延安市人民医院神经外科2012年10月-2015年10月收治自发性急性脑出血患者106例为脑出血组,另外选取我院同期健康志愿者为对照组,于出血后24 h内,3、7、14 d抽取静脉血,使用高速离心机分离上清液后用于检测CRP及IL-6水平。结果脑出血患者在出血后24 h内,3、7 d血清CRP与IL-6均较对照组明显增高（P〈0.05）;至14 d时血清CRP与IL-6水平与对照组比较,差异无统计学意义（P〉0.05）;不同血肿量间比较,发现血清CRP与IL-6水平在出血后24 h内,3、7 d均随着血肿量的增加而升高,在14 d时,不同血肿量间CRP与IL-6水平比较差异无统计学意义（P〉0.05）。结论 CRP及IL-6与脑出血的病理生理过程密切相关,且CRPIL-6水平对判定脑出血病情的严重程度有一定意义。Objective To explore the changes and clinical significance of serum CRP and IL- 6 in patients with acute spontaneous intracerebral hemorrhage. Methods 106 patients with acute spontaneous intracerebral hemorrhage in Yan＇an City People＇s Hospital of Neurosurgery from October 2012 to October 2015 were detected as cerebral hemorrhage group, healthy volunteers in the same time as control group, the level of CRP and IL-6 using high-speed centrifuge after within 24 hours, 3 days, 7 days, 14 days were detected. Results The level of serum CRP and IL-6 on patients of intracerebral hemorrhage was significantly higher than control group after within 24 hours, 3 days, 7 days（P〈0.05）; but there was no significant difference after 14 days（P〈0.05）; Comparison between different hematoma volume,serum CRP and IL-6 levels within 24 hours after the bleeding, 3 d, 7 d hematoma rises with the increase of the amount. There was no significant difference of the amount of CRP and IL-6 levels among hematoma in 14 days（P〈0.05）. Conclusion CRP and IL-6 are closely related to the pathophysiology of cerebral hemorrhage, and the levels of CRP and IL-6 have a certain significance on the determination of severity of cerebral hemorrhage.
