复方奥美拉唑胶囊治疗十二指肠溃疡疗效评价

摘　　要：目的观察复方奥美拉唑胶囊治疗十二指肠溃疡(DU)的疗效和安全性。方法采用随机法将80例DU患者均分为复方奥美拉唑胶囊试验组和进口奥美拉唑镁肠溶片对照组,均为20 mg/d,连服4周。在入组前、治疗2周末及4周末分别记录两组有关消化道临床症状并评分、行胃镜检查;抽吸胃液行p H值和表皮生长因子(EGF)含量测定;十二指肠溃疡边缘取材活检免疫组化法行表皮生长因子受体(EGFR)阳性表达率测定;比较两组不良事件发生率。结果在治疗2周末试验组、对照组临床症状评分总和分别为7.52±2.28、12.82±2.80;胃液p H值分别为5.73±0.43、4.90±0.56;EGF含量分别为(483.50±9.96)ng/L、(352.90±7.89)ng/L;EGFR阳性表达率分别为82.5%、75.0%,两组比较差异均有统计学意义(P<0.05)。在治疗4周末试验组、对照组临床症状评分总和分别为3.65±2.21、3.81±1.70;胃液p H值分别为8.24±0.17、8.31±0.21;EGF含量分别为(768.90±10.06)ng/L、(762.51±18.12)ng/L;EGFR阳性表达率分别为95.0%、92.5%;溃疡愈合率分别为85.0%、87.5%,两组比较差异均无统计学意义(P>0.05)。两组不良事件发生率均低。结论复方奥美拉唑胶囊能安全、有效的治疗DU,且起效更快。Objective To observe curative effect and safety of Compound Omeprazole Capsule in the treatment of duodenal ulcer. Methods 80 duodenal ulcer patients were equally divided into two groups,compound omeprazole capsule experimental group and imported omeprazole magnesium enteric- coated tablet control one,using random method. Dosage was 20 mg per day and continuously taken for 4 weeks. Symptoms of the digestive tract were recorded and scored,the position,quantity,shape,size and stage of duodenal ulcer were recorded by gastroscopy,values of p H and epidermal growth factor（ EGF） in gastric juice were measured and positive expression rates of epidermal growth factor receptor（ EGFR） were detected in duodenal ulcer margin biopsy specimen by immunohistochemical stain,before entering the group,2 weeks and 4weeks after taking medicine,respectively. Blood,urine,stool routine,liver and kidney function,electrolytes,fasting blood glucose and ECG were evaluated at each time to observe the incidence of severe event.Results Symptom score sums of two groups 2 weeks after taking medicine were 7. 52 ± 2. 28,12. 82 ± 2. 80,respectively. Values of p H of gastric juice were 5. 73 ± 0. 43,4. 90 ± 0. 56,respectively. Values of EGF were（ 483. 50 ± 9. 96） ng / L,（ 352. 90 ± 7. 89） ng / L,respectively.
