摘 要:目的 探究P16与Ki67的表达在子宫颈病变诊断中的应用.方法 采用回顾性分析的方法,选取2014年8月—2015年12月期间在该院妇科进行治疗的90例子宫颈病变患者作为该研究的对象,其中30例为低级别鳞状上皮内病变(LSIL)(包括CINI,扁平湿疣,尖锐湿疣),30例为高级别鳞状上皮内病变(HSIL)(CINII10例,CINIII20例)、30例子宫颈良性反应性病变(包括不成熟鳞化,上皮萎缩,修复性上皮增生)患者,采用免疫组化SP法对这90例患者的P16以及Ki67表达情况进行检测分析.结果经过HE染色分析,可以发现Ki67表达阳性细胞在宫颈鳞状上皮中的位置分布与子宫颈上皮内瘤变有着一定的相关性,LSIL多分布于下1/3的位置,CINII多分布于下1/3~2/3的位置,而CINIII多分布于〉2/3位置,子宫颈良性反应性病变分布于下1/3的基底和副基底层;P16的表达水平在LSIL、HSIL、子宫颈良性反应性病变中的差异有统计学意义(P=0.021〈0.05).且P16的表达水平与子宫颈上皮内瘤变的级别呈现出正相关的关系,差异有统计学意义(P=0.013〈0.05).结论 P16与Ki67的表达可水平可用于子宫颈良性反应性病变的甄别,还与子宫颈上皮内瘤变的进展联系密切,对P16以及Ki67表达水平的联合检测能够帮助子宫颈病变诊断,可以在临床医学中得以广泛的推广应用.Objective To discuss the application of the expression of P16 and Ki67 in diagnosis of cervical lesions. Methods90 cases of patients with cervical lesions treated in the department of gynaecology of our hospital from August 2014 to December 2015 were selected as the research object by the retrospective analysis method, among them, 30 cases were lowgrade squamous intraepithelial lesion(including CINI, condyloma, condyloma acuminatum), 30 cases were high-grade squamous intraepithelial lesion(10 cases were CINII, 20 cases were CINIII), 30 cases were benign reactive lesions of cervix(including immature squamous metaplasia, epithelial atrophy, reparative epithelial hyperplasia), the expression of P16 and Ki67 of the 90 cases of patients was detected and analyzed by SP immunohistochemical technique. Results The HE staining analysis discovered that the location distribution of Ki67 expression positive cells in the cervical squamous epithelium had a certain correlation with the cervical intraepithelial neoplasia, LSIL was more distributed in the location of lower 1/3, CINII was more distributed in the location of lower1/3~2/3, but CINIII was more distributed in the location〉2/3, the benign reactive lesions of cervix was distributed in the base of lower 1/3 and parabasal layer, the difference of the expression of P16 in the LSIL, HSIL and benign reactive lesions of cervix reached 0.021 with statistical significance, and the expression level of P16 had a positive correlation with cervical intraepithelial neoplasia levels and the difference value was 0.013 with statistical significance. Conclusion The expression levels of P16 and Ki67 can be used to discriminate the benign reactive lesions of cervix and have a close correlation with the evaluation of the cervical intraepithelial neoplasia, the combined detection of the expression levels of P16 and Ki67 contributes to the diagnosis of cervical lesions, which can be widely promoted and applied in the clinical medicine.
