3种不同手术方式子宫切除的临床分析

摘　　要：目的对腹腔镜全子宫切除术(LAVH)、经腹全子宫切除术(TAH)、阴式子宫切除术(TVH)3种子宫肌瘤治疗方法的效果进行分析探究。方法随机选取2013年6月—2015年6月在该院接受手术治疗的子宫肌瘤患者90例,按照随机数字表法分组,平均分为LAVH组、TAH组、TVH组,分别采用腹腔镜全子宫切除术(LAVH)、经腹全子宫切除术(TAH)、阴式子宫切除术(TVH)治疗,对比分析治疗效果情况。结果 LAVH组研究对象的手术时间、术中出血量、术后排气时间以及术后住院时间(37.3±5.6)min、(55±10.5)m L、(15±3.5)h、(3.4±1.1)d均优于TAH组(46.4±6.8)min、(80±11)m L、(24±10)h、(5.5±2.5)d及TVH组(47.4±5.7)min、(70±8.5)m L、(24±3)h、(5.6±1.9)d,差异有统计学意义(P<0.05)。结论对子宫肌瘤患者采用腹腔镜下全子宫切除术治疗,创伤性小,出血少,恢复快,临床疗效显著,所以在选择治疗方式是根据患者的实际情况应尽量选择腹腔镜下全子宫切除术加以治疗。Objective To analyze and study the effect of three treatment methods of hysterectomy including the total laparoscopic hysterectomy, abdominouterectomy and transvaginal hysterectomy. Methods 90 cases of patients with myoma of uterus treated in our hospital from June 2013 to June 2015 were selected and randomly divided into the LAVH group, TAH group and TVH group, and the three groups were respectively treated with the total laparoscopic hysterectomy, abdominouterectomy and transvaginal hysterectomy, and the treatment effects were compared and analyzed. Results The operative time, intraoperative blood loss, postoperative exhaust time and postoperative length of stay in the LAVH group were better than those in the TAH group and TVH group, [（37.3±5.6） min,（55±10.5） m L,（15±3.5） h,（3.4±1.1） d vs（46.4±6.8） min,（80±11） m L,（24±10） h,（5.5±2.5） d and（47.4±5.7） min,（70±8.5） m L,（24±3） h,（5.6±1.9） d], and the differences had statistical significance（P 〈0.05）. Conclusion The total laparoscopic hysterectomy in treatment of patients with myoma of uterus has small trauma, less bleeding and rapid recovery and the clinical curative effect is obvious, therefore, we had better choose the total laparoscopic hysterectomy according to the actual conditions of patients when we choose the treatment method.
