摘 要:目的:代谢综合征(metabolic syndrome,MS)可能与乳腺癌的发生相关。本研究探讨MS在乳腺疾病病人中的发生情况,及与乳腺癌发生之间的关系。方法:选取2012年12月至2015年3月在上海瑞金医院乳腺疾病诊治中心手术的乳腺疾病病人,检测并收集病人的体质量指数(body mass index,BMI)、血压、血脂、血糖和临床资料,比较其在乳腺良、恶性疾病病人中的差异。结果:共有2 816例病人,乳腺良性和恶性病人分别为1 500例和1 316例,其BMI分别为(22.8±3.1)和(23.4±3.2)(P=0.018);伴MS人数分别有131例(8.7%)和191例(14.5%)(P〈0.001)。单因素分析显示,MS和肥胖等代谢紊乱在乳腺良、恶性疾病组间差异有统计学意义(P〈0.05)。多因素分析显示,代谢紊乱中仅肥胖(P=0.010)是乳腺癌发生的独立影响因素。与BMI〈25.0的女性相比,肥胖女性具有更高的乳腺癌患病风险(OR=1.28,95%CI=1.06~1.56)。结论:有14.5%的乳腺癌病人存在MS,较乳腺良性疾病病人合并MS高;肥胖可能增加乳腺癌的发生,值得进一步研究证实。Objective Metabolic syndrome (MS) may be associated with development of breast cancer. In this study, we aim to evaluate the prevalence of MS in patients with breast disease and the relationship between MS and breast cancer. Methods The patients with breast disease who had surgery in Comprehensive Breast Health Center, Department of Surgery of Ruijin Hospital from Dec 2012 to Mar 2015 were enrolled. Measurement was made and recorded including body mass index(BMI), blood pressure, blood lipids and glucose combined with clinical factors. Differences was compared between benign breast diseases and brease cancer. Results A total of 2 816 cases were enrolled including 1 500 cases with benign breast diseases and 1 316 cases with breast cancer. BMI was (22.8~3.1) in benign breast disease group and (23.4+3.2) in breast cancer group (P=0.018). MS was diagnosed in 131 (8.7%) cases in benign breast diseases group and 191(14.5%) cases in breast cancer(P〈0.001). Univariate analysis showed that the difference of BM1 and MS between benign breast disease group and breast cancer groups was statistically significant (P〈O.05). It was shown by multivariate analysis that obesity was independent risk factor of breast cancer alone among the factors of metabolic abnormality(P=0.010). Compared with cases with BMI 〈25.0, obesity cases had higher breast cancer incidence risk (OR=1.28, 95% CI=1.06-1.56). Conclusions MS was found in 14.5% breast cancer patients more than in the patients with benign breast disease. Obesity may increase breast cancer incidence which needs further study.
