游离脂肪移植修复乳腺癌保乳术后局部凹陷畸形的临床研究

摘　　要：目的：探讨应用游离脂肪移植技术修复乳腺癌保乳手术后造成的乳房局部缺损畸形的临床效果。方法：选取2012年1月-2015年6月于本科接受乳腺癌保乳手术后又接受脂肪移植修复局部凹陷畸形的45例患者作为试验组,只接受保乳手术未行修复手术的55例患者作为对照组,试验组通过自体脂肪游离移植的方法,从患者腹部抽出脂肪组织经过处理后注射至患侧乳房的凹陷部位以达到填充的效果,试验组脂肪移植术后患者随访3个月~1年,比较两组患者的满意率。结果：试验组通过第一次游离脂肪移植后对乳房外形满意者22例,占48.89%,第二次行游离脂肪移植后对乳房外形满意者17例,占37.78%,总体满意率86.67%;对照组对乳房外形满意者13例,总体满意率为23.63%,两组患者满意率比较差异有统计学意义（P〈0.01）。结论：脂肪移植技术效果可靠,安全性高,在临床上可用于修复乳腺癌术后乳房的局部凹陷缺损。Objective： To investigate clinical effect of free fat transplantation used for repairing the local depression deformity after breast conserving surgery for breast cancer.Method： From January 2012 to June 2015, 45 patients who got free fat transplantation after breast conserving surgery for breast cancer in our department were selected as the experimental group, and 55 patients who only got breast conserving surgery for breast cancer as the control group.Patients in the experimental group were treated by autologous fat transplantation, abdominal fat was extracted to filling the depression deformity of breast after surgery.Patients in the experimental group were followed up for 3 months to 1 years after free fat transplantation.The satisfaction rate of two groups were compared.Result： Forty-five cases of breast conserving surgery for breast cancer in experimental group, 22 cases were satisfied for breast appearance after the first free fat transplantation, accounted for 48.89%, 17 cases were satisfied for breast appearance after the second free fat transplantation, accounted for 37.78%, the overall satisfaction rate was 86.67%.55 cases of breast conserving surgery for breast cancer in the control group, 13 cases were satisfied for breast appearance without fat transplant operation, accounted for 23.63%.There were significant differences in the satisfaction rate of two groups, the difference was statistically significant （ P〈0.01 ） .Conclusion： Free fat transplantation has the advantages of significant effect and high safety, can be used to repairing the local depression deformity after breast cancer surgery in clinic.
