纳他霉素滴眼剂治疗真菌性角膜炎的疗效评估

摘　　要：目的观察纳他霉素滴眼剂治疗真菌性角膜炎的临床效果,探讨真菌性角膜炎药物治疗的作用机制,以提高真菌性角膜炎患者临床疗效。方法选取2013年1月-2015年10月医院诊治的80例真菌性角膜炎患者资料,采用随机对照方法将患者分为对照组和试验组,每组40例,对照组采用伏立康唑滴眼液治疗,试验组采用纳他霉素滴眼剂局部滴眼治疗,比较两组疗效。结果试验组治疗总有效率为95%,显著高于对照组（P〈0.05）;试验组治疗后复发率为5%,显著低于对照组（P〈0.05）;两组患者治疗前视力≤0.1、0.1-0.3以及≥0.3者差异无统计学意义;试验组治疗后视力≥0.3,显著多于对照组（P〈0.05）;试验组视力≤0.1、0.1-0.3者显著少于对照组（P〈0.05）;两组患者治疗前眼部评分差异无统计学意义;试验组治疗后3d、7d、14d、28d眼部评分,显著低于对照组（P〈0.05）。结论真菌性角膜炎患者采用纳他霉素滴眼剂局部滴眼治疗效果理想,药物不良反应发生率低,能够改善患者视力,值得推广应用。OBJECTIVE To observe the clinical effect of natamycin eye drops on fungal keratitis,and to discuss the mechanism of drug treatment so as to improve the clinical efficacy for patients with fungal keratitis.METHODS A total of 80cases（80eyes）of patients with fungal keratitis admitted from Jan.2013 to Oct.2015 were analyzed.Randomized controlled trials were conducted to divide them into control group and experiment group with 40 cases each.The control group received voriconazole eye drops,while the experimental group used natamycin of eye drops for topical treatment.The clinical effects of the two groups were compared.RESULTS The total effect rate of the experimental group was 95%,which was significantly higher than that of the control group（P〈0.05）;the relapse rate of the experiment group was 5%,which was significantly lower than the control group（P〈0.05）;the differences between two groups of patients with vision≤0.1,0.1-0.3and≥0.3were not significant;the cases of vision ≥0.3in experimental group after treatment were significantly more than the control group（P〈0.05）;while visual acuity ≤0.1,0.1-0.3in experiment group were significantly less than the control group（P〈0.05）;the differences of eye scores between two groups of patients before difference treatments were not significant;the eye scores of the experimental group after treatment in 3d,7d,14 d,28dwere significantly lower than those of the control group（P〈0.05）.CONCLUSION Natamycin eye drops have great effect and low incidence of adverse drug reactions on patients with fungal keratitis.They can improve eye vision so can be applied widely.
