吻合器痔上黏膜环形切除术对混合性痔的远期临床疗效研究

摘　　要：目的:探讨吻合器痔上黏膜环形切除术治疗混合性痔的远期临床疗效。方法:选取我院收治的混合性痔患者90例,随机分为对照组及实验组,对照组以传统痔体剥离结扎术方案治疗,实验组以吻合器痔上黏膜环形切除术治疗。比较两组患者手术时间、出血量、住院时间、止痛药量、术后出血量、瘢痕增生、肛门功能及术后6到12个月的复发率等情况,其数据结果应用统计学软件SPSS 17.0处理。结果:与对照组比较,实验组术中时间、出血量、住院时间、止痛药量、瘢痕增生及复发率均低于对照组(P<0.05),实验组肛门功能高于对照组(P<0.05)。结论:吻合器痔上黏膜环形切除术对肛管黏膜损伤小,痔体切除完全,愈合迅速,吻合钉固定可靠,瘢痕规整,愈后康复痛苦小,远期复发率低,临床疗效理想,可作为临床治疗的首选方案。Objective： To investigate the long term clinical effect of procedure for prolapse and hemorrhoids with stapler in the treatment of mixed hemorrhoids. Methods： 90 mixed hemorrhoids patients from our hospital were selected and randomly divided into the control group and the experiment group. The patients in control group were treated with traditional hemorrhoid ligation peeling treatment program, and the patients in experimental group were given the annular stapler hemorrhoid mucosal resection. The operation time,amount of bleeding, hospitalization time, analgesic dosage, postoperative bleeding, scar hyperplasia, anal function and recurrence rate were compared between two groups. The data was analyzed using statistical software SPSS 17.0. Results： Compared with the control group, the operation time, amount of bleeding, hospitalization time, analgesic dosage, postoperative scar hyperplasia and recurrence rate were lower in experiment group（P〈0.05）, and the anal function were higher in experiment group（P〈0.05）. Conclusions： The procedure for prolapse and hemorrhoids with stapler can obtain little injury on anal mucosa, and complete resection of hemorrhoids and quick heal with reliable anastomotic nail fixation and regular scar, and its long-term recurrence rate is low and the clinical curative effect is ideal, so it can be taken as the first choice for clinical treatment of mixed hemorrhoids.
