低分子肝素治疗急性胰腺炎临床疗效的Meta分析

摘　　要：目的:系统评价低分子肝素(low molecular weight heparin,LMWP)治疗急性胰腺炎(acute pancreatitis,AP)临床疗效。方法:计算机检索Embase、Pub Med、VIP、CNKI、CBM、万方数据库,查找有关低分子肝素治疗急性胰腺炎的随机对照实验(randomized controlled trials,RCTs),并查找其参考文献,检索时限为2000年至2015年10月,由2名评价者独立进行文献提取资料并评估文献质量,采用Rev Man 5.3软件进行合并数据并行统计学分析。结果:最终纳入21篇文献,研究对象实验组共968例,对照组940例。Meta分析结果示,低分子肝素组治疗AP较对照组能提高治愈率(RR=1.46,95%CI=1.18~1.81)、降低重症化率(RR=0.38,95%CI=0.25~0.58)、改善APACHE-Ⅱ得分(MD=-2.01,95%CI=-3.32^-0.70)、降低并发症发生率(RR=0.53,95%CI=0.44~0.63)、降低死亡率(RR=0.34,95%CI=0.24~0.48)、缩短住院天数(MD=-7.57,95%CI=-10.35^-4.79)。结论:基于现有Meta分析结果表明,低分子肝素联合常规内科治疗对AP有较好的疗效。Objective：To evaluate the effectiveness of low molecular weight heparin（LMWH）for treating acute pancreatitis（AP）by Meta analysis. Methods ：Embase,Pub Med,VIP,CNKI,Wan Fang, CBM databases were searched by computer. the randomized controlled trials（RCTs）from 2000 to 2015 were collected,and the relevant references of the included studies were also retrieved.Data screening and extraction were completed by two reviewers independently. Meta-analyses were conducted by Rev Man5.3 software.Results ：A total of 21 studies involving 1 908 participants were included. The Meta analysis results showed that compared with controlled group,the treatment group was superior in the following aspects with significant differences：increasing the curative rate（RR=1.46,95%CI=1.18 to1.81）,reducing the intensive rate（RR=0.38,95%CI=0.25 to 0.58）,reducing the APACHE-Ⅱ grade after treatment（MD=-2.01,95%CI=-3.32 to-0.70）,reducing the complication rate（RR=0.53,95%CI=0.44 to 0.63）,reducing the mortality rate（RR=0.34,95%CI=0.24 to 0.48）and shortening the length of hospital stay（MD=-7.57,95%CI=-10.35 to-4.79）. Conclusion：LMWH combined with conventional treatment is effective in treating AP.
