小柴胡汤辅助化疗对晚期肺癌患者炎性因子与免疫功能的影响

摘　　要：目的：探讨小柴胡汤辅助化疗对晚期肺癌患者炎性因子与免疫功能的影响。方法：选择2012年5月—2014年11月间接受住院治疗的晚期肺癌患者112例为研究对象,采用随机数字表法分为观察组及对照组各56例。对照组患者接受单纯化疗,观察组患者接受小柴胡汤辅助化疗,比较两组炎性因子、免疫功能、治疗效果、不良反应。结果：近期疗效：观察组有效率、疾病控制率均明显高于对照组（46.43%vs 23.21%,78.57%vs60.71%）（χ2=4.223~6.648,P〈0.05）;炎性因子：观察组患者血清C反应蛋白（CRP）、肿瘤坏死因子-α（TNF-α）、白介素-6（IL-6）水平低于对照组患者（t=14.101~17.437,P〈0.05）;免疫功能：观察组CD4＋、CD4＋/CD8＋水平明显高于对照组,CD8＋水平明显低于对照组（t=3.185~7.007,P〈0.05）;不良反应：观察组白细胞减少、血小板减少、血红蛋白下降、肾功能损害、恶心呕吐等不良反应明显低于对照组（χ2=3.953~6.038,P〈0.05）。结论：小柴胡汤辅助化疗有助于缓解晚期肺癌患者全身炎症状态,改善免疫功能,减少不良反应,提高治疗效果。Objective： To study the effect on inflammatory factors and immune function of Xiaochaihu Decoction assisting chemotherapy for patients with advanced lung cancer. Methods： A total of 112 cases patients with advanced lung cancer from May 2012 to November 2014 were divided into observation group and control group. The control group was given chemotherapy therapy and the observation group given Xiaochaihu Decoction and chemotherapy. The inflammatory cytokines,immune function,therapy effect and adverse reactions were compared between two groups. Result： Recent curative effect： observation group＇s efficient and disease control rates were significantly higher than those of the control group（ 46. 43% vs 23. 21%,78. 57% vs 60. 71%）（ χ2= 4. 223 ~ 6. 648,P〈0. 05）. Inflammatory factors： the observation group＇s serum CRP,TNF- α and IL- 6 levels were significantly lower than those of the control group（ t = 14. 101 ~17. 437,P〈0. 05）. Immune function： the observation group＇s CD4＋and CD4＋/ CD8＋levels were significantly higher than the those of the control group and CD8＋significantly lower than the control group＇s（ t = 3. 185 ~ 7. 007,P〈0. 05）.Adverse reactions： the observation group＇s leukopenia,thrombocytopenia,decreased hemoglobin,kidney damage,nausea and vomiting were significantly lower than the control group＇s（ χ2= 3. 953 ~ 6. 038,P〈0. 05）. Conclusion： Xiaochaihu Decoction can help to ease the inflammatory stat,improve the immune function,reduce the adverse reaction and improve the therapy effect when assisting chemotherapy.
