摘 要:目的 探讨简短认知行为治疗对失眠症的疗效.方法 收集2015年3月至2015年7月解放军第一八七医院神经内科的失眠症患者48例.采用随机数字表法分为简短认知行为治疗组(BCBT组)及佐匹克隆组(药物组),两组均为24例,共干预4周;对比两组睡眠参数、汉密顿焦虑量表(HAMA)分值、失眠严重程度指数(ISI)分值差异.结果 干预结束时,BCBT组与药物组入睡潜伏期[(39.79± 15.50) min vs(34.79±20.61) min]、睡眠效率[(84.28±7.06)% vs (84.89±9.10)%]及入睡后觉醒次数[(1.13±0.80)次vs (1.17±0.70)次]差异无统计学意义(P>0.05),BCBT组总睡眠时间[(324.58±36.77) min]、在床时间[(384.79±25.22) min]、HAMA分值[(13.29±3.65)分]、ISI分值[(14.42±4.19)分]显著小于药物组[(348.54±43.53) min、(410.83±28.16) min、(15.96±4.29)分、(17.13±4.77)分],差异具有统计学意义(P<0.05);两组治疗后上述指标与治疗前相比,均差异有统计学意义(P<0.05).结论 BCBT可改善失眠症状,降低焦虑水平及失眠严重程度,是临床上值得推广的治疗方法.Objective To explore the effects of brief cognitive behavioral therapy (BCBT) on clinical efficacy in insomnia.Methods 48 cases of insomnia were collected in the department of neurology in 187th Hospital of PLA from March to July 2015.They were divided into brief cognitive behavioral therapy (BCBT group) and Zopiclone group (drug therapy group) by random number table method,24 cases in each group,and the treatment continued for 4 weeks.The sleep parameters,Hamilton Anxiety Scale (HAMA) scores and Insomnia Severity Index (ISI) scores were recorded and compared in two groups.Results At the post-treatment time point,the difference of sleep onset latency((39.79±15.50) min vs (34.79±20.61)min),sleep efficiency((84.28±7.06) vs (84.89±9.10)),number of awakenings ((1.13±0.80) vs (1.17±0.70))was not statistically significant (P〉0.05) between BCBT group and drug therapy group.In BCBT group,total sleep time((324.58±36.77) min),time in bed((384.79±25.22) min),HAMA score (13.29±3.65) and ISI score (14.42±4.19) were significantly lower than those in drug therapy group((348.54±43.53) min,(410.83 ±28.16) min,(15.96±4.29),(17.13±4.77)) (P〈0.05).The differences of all indicators between pre-and post-treatment were statistically significant in both groups(P〈0.05).Conclusion BCBT can improve insomnia symptoms,and reduce the level of anxiety and insomnia severity.It should be popularized in clinic.
