乳腺分叶状肿瘤的临床病理特点及治疗体会

摘　　要：目的探讨乳腺分叶状肿瘤的临床病理特点及治疗体会。方法回顾性分析38例乳腺分叶状肿瘤的临床资料,分析其临床病理特征,并采取不同手术治疗方式,观察临床效果和复发率。结果本组38例患者中,乳腺分叶状肿瘤主要为单侧单发无痛包块,无包膜,肿块直径多小于5 cm,病理类型以良性居多(P<0.05)。38例患者共有8例复发,总复发率为21.1%,其中良性叶状肿瘤复发5例(13.2%),交界性叶状肿瘤复发2例(5.3%),恶性叶状肿瘤复发1例(2.6%)。接受乳腺局部切除术的患者复发率明显高于局部扩大切除术和全切术者,差异有统计学意义(P<0.05)。结论乳腺分叶状肿瘤临床病例特点主要为单侧单发无痛包块,无包膜,手术治疗是其主要治疗手段,手术效果与手术方式密切相关。Objective To investigate the clinicopathological characteristics and treatment of phyllodes tumor of the breast.Methods The clinical data of 38 cases of phyllodes tumor of the breast were reviewed,and the clinicopathological characteristics were analyzed. Patients were treated with different surgical procedures,and the clinical effects and recurrence rates were observed.Results Among the 38 cases,most of the phyllodes tumor of the breast were unilateral solitary painless mass,with no capsule and less than 5 cm in tumor diameter,and the pathological types were mostly benign（P〈0.05）. Among the 38 cases,there were 8 cases of recurrence,with the recurrence rate of 21.1%. Of the 8 cases of recurrence,5（13.2%）were benign phyllodes tumor,2（5.3%）were borderline phyllodes tumor,and the other 1（2.6%）was malignant phyllodes tumor. The recurrence rate of patients with local resection of breast was significantly higher than that of patients with extended local resection of breast and mastectomy（P〈0.05）.Conclusion Phyllodes tumor of the breast is clinically featured by unilateral and solitary painless mass,with no capsule,which is mainly treated by surgical procedures,and the operation effect is closely related to surgical procedures.
