摘  要:目的探讨雌激素和雌、孕激素联合应用所致大鼠子宫肌瘤后的免疫功能变化及其机制。方法选取SPF级雌性SD大鼠90只,编号后采用随机数字表法分为空白组、雌激素组和雌+孕激素组各30只,采用苯甲酸雌二醇对雌激素组和雌+孕激素组进行肌肉注射连续3个月(0.2 mg/只,3次/周),雌+孕激素组在造模3个月后加用黄体酮1.5 mg/只肌注(2次/周,连续1个月)。观察比较三组大鼠子宫组织平滑肌厚度、子宫重量、卵巢重量,以及E_2水平、ER评分、DR评分,测定T细胞亚群。结果子宫组织平滑肌厚度值雌+孕激素组>雌激素组>空白组且组间差异均具有统计学意义(P<0.05)。雌+孕激素、雌激素组大鼠的子宫重量在造模第3个月、4个月较空白组显著的增加(P<0.05)、卵巢重量较空白组显著的降低(P<0.05)。雌+孕激素、雌激素组大鼠的E2水平、ER评分、PR评分显著高于空白组(P<0.05),雌+孕激素组的E2水平、ER评分、PR评分显著高于雌激素组(P<0.05)。雌+孕激素、雌激素组大鼠的CD3+、CD4^+、CD4^+/CD8^+水平显著低于空白组(P<0.05)、CD8^+显著高于空白组(P<0.05),雌+孕激素组的CD3+、CD4^+、CD4^+/CD8^+水平显著低于雌激素组(P<0.05)。结论雌激素和雌、孕激素联合应用所致大鼠子宫肌瘤模型免疫功能显著低下,激素受体水平可能是其主要影响因素之一。Objective To investigate the changes and mechanisms of the immune function of the rat uterine leiomyoma model induced by estrogen combined with estrogen and progesterone. Methods 90 female SD rats of SPF grade were selected, number by random number table method for blank control group, estrogen group and estrogen plus progesterone group with 30 rats in each group. The estrogen group and estrogen plus progesterone group were treated by intramuscular injection for 3 consecutive months with estradiol benzoate(0.2 mg / mouse,3 times / week). The estrogen plus progesterone group in rats 3 months after plus progesterone 1.5 mg only intramuscularly(2 times / week, for months). The uterine smooth muscle thickness value,uterine weights,ovary weights were compared of three groups, as well as E2 level, ER and PR score,and T cell subsets were measured. Results The uterine smooth muscle thickness of estrogen plus progesterone group 〉estrogen group〉blank control(P〈0.05). The uterine weights of the estrogen group and estrogen plus progesterone group at 3 and 4 months were significantly increase and heavier than those of the blank control group(P〈0.05), while the ovary weights were significantly decreased.The levels of E_2, ER score and PR score of the estrogen group and estrogen plus progesterone group were significantly higher than those in the blank control group(P〈0.05). The levels of CD3+,CD4+and CD4+/ CD8+cells of the estrogen group and estrogen plus progesterone group were significantly lower than those in the blank control group(P〈0.05), while CD8+cells were significantly higher than that of the blank control group(P〈0.05). The CD3+,CD4+and CD4+/ CD8+cells of the estrogen plus progesterone group were significantly lower than those in the estrogen group(P〈0.05). Conclusion Estrogen, estrogen and progestin combination induced uterine leiomyoma rat model showed an impaired immune function and the hormone receptor level may be one of the causingfactor.
