牙龈环切辅助牙周炎患牙正畸压低对牙周炎患牙预后的影响

摘　　要：目的探讨牙龈环切辅助牙周炎患牙正畸压低对牙周炎患牙预后的影响。方法选取伴有附着丧失的牙周炎患者86例,按照随机数字表法分为研究组和对照组各43例。所有患者牙周基础治疗后进行正畸治疗。研究组在排齐整平阶段配合牙龈环切治疗,对照组不行牙龈环切。对2组的牙周状况、IL-1β及TNF-α水平、牙根长、牙骨与牙槽嵴顶间距离(CEJAC)、冠根比、牙槽骨吸收比例(ABLR)、复发率等指标变化进行比较。结果 2组治疗前、后的牙周探诊深度(PD)、平均附着丧失(AL)、菌斑指数(PLI)、龈沟出血指数(SBI)、探诊出血(BOP)相比有显著差异(P<0.05),且2组治疗后的PD、AL、PLI、SBI、BOP相比有显著差异(P<0.05)。研究组治疗后IL-1β、TNF-α水平显著低于治疗前(P<0.05),且显著低于对照组治疗后(P<0.05)。2组患者治疗前、后牙根长、CEJ-AC、冠根比、ABLR相比有显著差异(P<0.05),且2组治疗后牙根长、CEJAC、冠根比、ABLR相比有显著差异(P<0.05)。研究组复发率为16.28%,显著低于对照组的41.86%(P<0.05)。结论牙龈环切辅助牙周炎患牙正畸压低可有效提高牙周状况,减少炎症因子的释放,提高周围牙槽骨高度和治疗效果。Objective To explore the effect of circumferential supracrestal fibrotomy combined with orthodontic depression on prognosis of patients with periodontitis teeth. Methods A total of 86 patients with periodontitis were selected and randomly divided into research group and control group,43 cases in each group. All patients were treated with orthodontic treatment after periodontal basic treatment. Research group was treated with circumferential supracrestal fibrotomy during aligning and leveling stage of orthodontic treatment,while the control group was not given circumferential supracrestal fibrotomy. The periodontal status,IL-1β and TNF-α,root length,distance between teeth and alveolar bone crest（ CEJ-AC）,crown root ratio,proportion of alveolar bone absorption（ ABLR）and recurrence rate were compared between the two groups. Results There were significant differences in the periodontal probing depth（ PD）,attachment loss（ AL）,plaque index（ PLI）,sulcular bleeding index（ SBI） and bleeding on probing（ BOP） before and after treatment in both groups（ P〈0. 05）,and the PD,AL,PLI,SBI,BOP of the research group after treatment were significantly better than those of the control group（ P〈0. 05）. The levels of serum IL-1β and TNF-α of research group after treatment were significantly lower than those before treatment（ P〈0. 05）,and the level of IL-1β and TNF-α were also significantly lower than those after treatment in control group（ P〈0. 05）. There were significant differences in the root length,CEJ-AC,crown root ratio and ABLR before and after treatment in both groups（ P〈0. 01）,and the root length,CEJ-AC,crown root ratio and ABLR after treatment in the research group were significantly better than those in control group（ P〈0. 05）. The recurrence rate of research group was 16. 28%,which was significantly lower than41. 86% of the control group（ P〈0. 05）.
