宫颈人乳头瘤病毒联合液基细胞学检测在宫颈疾病筛查中的价值分析

摘　　要：目的：探讨宫颈人乳头瘤病毒（HPV）和液基细胞学（TCT）检测在不同级别宫颈病变筛查中的临床应用价值。方法选取就诊于该院妇科门诊的5419例患者同时行 HPV 和 TCT 检测，对 HPV 阳性或（和）TCT 检测异常，或 HPV 和 TCT 检测正常但妇检高度可疑病变的1936例患者予以阴道镜下宫颈组织活检并行病理检查。根据 HPV、TCT 检测及阴道镜活检病理结果分析，比较 HPV、TCT 检测在宫颈病变筛查中的价值。结果5419例患者中，HPV 感染率为20％，感染率由高到低的前五种亚型依次为 HPV16、52、18、58、53；＞30～≤40岁组 HPV 感染率最高；TCT 阳性率随年龄段增长而升高；HPV 感染率随 TCT 分级升高而升高；HPV 联合 TCT 检测灵敏度为99．23％，特异度为62．05％，单独使用 HPV 检测的灵敏度为72．43％，特异度为56．56％，单独使用 TCT 检测的灵敏度为73．51％，特异度为47．09％。结论HPV 联合 TCT 检测在宫颈病变筛查中具有较高的灵敏度和特异度，值得临床推广应用。Objective To investigate the application value of human papillomavirus （HPV）and cervical thin-prep cytologic test （TCT）testing in the screening of different grades of cervical lesions.Methods 5 419 women who took cervical screening in our hospi-tal all underwent HPV testing and the TCT.1 936 women with positive screening results of HPV or/and positive screening results of TCT,or abnormal cervical appearance although with normal HPV and TCT,were further given colposcopy and biopsy.Results Of 5 419 patients,HPV infection rate was 20%,and the infection rates from high to low in the first five subtypes were HPV16,52,18,58, and 53.HPV infection rate was highest in 〉30 ~≤40 years old group.The positive rate of TCT increased with age.HPV infection rate increased with severity of TCT results.The detection sensitivity of HPV combined with TCT was 99.23%,and the specificity was 62.05%.The sensitivity of HPV detection alone was 72.43%,and the specificity was 56.56%.The sensitivity of single TCT detection alone was 73.51%,and the specificity was 47.09%.Conclusions High-risk type HPV combined with TCT testing,increases the sen-sitivity and specificity of the diagnosis of cervical lesions,showing good clinical application value.
