摘　　要：目的 评估CYP4A11基因多态性及其与脑梗死发病及梗死后血管事件的相关性。方法 采用基质辅助激光解吸附电离/飞行时间质谱法（MDLDI-TOF-MS）对396例脑梗死患者及378例对照者的CPYP4 A11rs2269231、CPYP4A11rs9333025、CYP4F2rs3093135及CYP4F2rs2108622 SNPs进行检测。并对所有的脑梗死患者随访12月。结果 脑梗死组CPYP4 A11rs9333025 GG基因型频率发布明显高于对照组（P〈0.001）。CPYP4 A11rs9333025 GG与脑梗死的发病密切相关（OR=1.82,95%CI：1.24~5.04,P=0.016）。rs9333025 GG基因型患者在随访期内脑梗死复发率及其他血管事件发生率明显高于rs9333025 AG基因型患者（P=0.04）。rs9333025 GG为脑梗死后血管事件发生的独立危险因素（RR=1.87,95%CI：1.16~5.36,P=0.003）。结论 rs9333025 GG基因型增加了脑梗死的易感性和脑梗死后血管事件发生的风险。Objective To evaluate the associations between 4 single-nucleotide polymorphisms （ SNPs ） from the two genes and ischemic stroke as well as atherothrombotic events after stroke .Methods Three hundred and ninety-six patients with ischemic stroke and 378 controls were genotyped for 4 SNPs of the CYP4A11 gene （rs2269231 and rs9333025） and CYP4F2 gene （rs2108622 and rs3093135 ） using the method of matrix-assisted laser desorption/ionization time of flight mass spectrometry （ MALDI-TOF MS） .All patients were followed up 12-months for atherothrombotic events .Results The frequency of GG genotype of rs 9333025 was significantly higher in the ischemic stroke patients than that in controls （ P〈0.001 ） .Multiple logistic regression analysis showed that rs 9333025 GG was significantly associated with higher risk of ischemic stroke （ OR =1.82 ,95%CI =1.24~5.04 ,P =0.016 ） .The rs9333025 GG was significantly associated with higher atherothrombotic events than rs 9333025 AG during 12 months after stroke （ P =0.04 ） .Multi-ple Cox regression analysis revealed that the rs 9333025 GG was an independent risk factor for atherothrombotic events after adjustment for confounding factors （ RR =1.87 ,95%CI =1.16-5.36 ,P =0.003 ） .Conclusion rs9333025 GG genotype increases susceptibil-ity to ischemic stroke ,and is associated with high frequencies of atherothrombotic events in stroke patients .
