内镜逆行胰胆管造影治疗胆胰疾病在基层医院中的应用

摘　　要：目的探讨基层医院内镜逆行胰胆管造影(endoscopic retrograde cholangiopancreatography,ERCP)对胆源性胰腺炎、胆管梗阻疾病的诊断与治疗的价值。方法回顾性分析我院2009年12月至2014年3月收治的90例诊断与治疗性ERCP的临床资料。结果 90例ERCP中,胆管结石者62例,胆总管良性狭窄8例,胆总管肿瘤10例,胆总管囊肿1例,十二指肠乳头炎1例,壶腹癌3例;单纯ERCP2例,内镜下乳头括约肌切开取石(EST)80例,插管困难行十二指肠乳头开窗术14例,放置鼻胆引流管71例,放置塑料支架16例。手术成功88例,手术失败2例,成功率98%,均未发生严重并发症。结论通过ERCP相关技术可清晰地观察胆总管形态,对胆管疾病的诊断以及梗阻性胆管疾病病因治疗有重要价值。Objective To investigate the diagnostic and therapeutic value of endoscopic retrograde cholangiopancreatography （ERCP） for cholecystopancreatic disease at primary hospitals .Methods Clinical data of diagnostic and therapeutic ERCP of 90 pa-tients from December ,2009 to March,2014 were retrospectively reviewed .Results There were 62 cases with choledocholithiasis ,13 choledochal benign stricture ,10 cholangiocarcinoma ,1 choledochocele ,1 duodenal papillitis and 3 duodenal ampulla carcinoma .There were 2 cases treated with ERCP ,62 received EST ,14 cases who were difficult intubation received duodenal papilla fenestration ,71 cases treated with ENBD and 16 cases received plastic stent .The operation was success in 88 cases and failure in 2 cases.The surgical suc-cess rate was 98%with no serious complications .Conclusion ERCP not only can observe the choledoch clearly but also plays an im-portant role in the diagnosis of biliary duct disease and therapy of biliary pancreatitis .
