和肽素对颅内外动脉狭窄的影响

摘　　要：目的 探讨和肽素对颅内外动脉狭窄的影响。方法 选择2015年1~12月在我院神经内科住院治疗行头颈部CT血管成像检查的患者628例为研究对象。其中颅外动脉狭窄患者154例（颅外组）,颅内动脉狭窄患者131例（颅内组）,无狭窄患者343例（对照组）。分析和肽素与颅内动脉狭窄及颅外动脉狭窄的关系。结果 颅内组和颅外组患者平均年龄、收缩压、舒张压以及和肽素水平均显著高于对照组,高血压、糖尿病、冠心病史、目前吸烟、饮酒、代谢综合征、高脂血症发生率显著高于对照组,差异有统计学意义（P〈0.01）;HDL-C水平显著低于对照组,差异有统计学意义（P〈0.01）。和肽素〉403.9 pg/m L是颅内动脉狭窄的独立危险因素,和肽素〉611.3 pg/m L是颅外动脉狭窄的独立危险因素。结论 和肽素是颅内外动脉狭窄的独立危险因素,尤其是对颅内动脉狭窄影响更大。Objective To discuss effect of peptide hormone on internal and external carotid artery stenosis. Methods 628 cases detected with head and neck CT angiography from Jan to Dec 2015 were selected as subjects. 154 cases were with external carotid artery stenosis（external group）, 131 cases were with internal carotid artery stenosis（internal group）, and 343 cases were no carotid artery stenosis（control group）. Correlation between peptide hormone with internal and ex- ternal carotid artery stenosis was analyzed. Results Mean age, systolic blood pressure, diastolic blood pressure, and peptide hormone levels of external group and internal group were higher than control group, and history of hypertension, diabetes mellitus, coronary heart disease, smoking, alcohol consumption, metabolic syndrome, hyperlipidemia rates were higher than control group（P〈0.01 ）;HDL levels were lower than control group（P〈0.01 ）. Peptide hormone 〉403.9 pg/mL was independent risk factor for internal carotid artery stenosis, and peptide hormone〉611.3 pg/mL was independent risk factor for external carotid artery stenosis. Conclusion Peptide hormone is risk factors for internal and external carotid artery stenosis, especially for internal carotid artery stenosis.
