食管癌患者血清白细胞介素和急性时相反应蛋白水平及其临床意义

摘　　要：目的 探讨食管癌患者血清细胞因子白细胞介素-8（IL-8）、白细胞介素-10（IL-10）、白细胞介素-18（IL-18）和C反应蛋白、转铁蛋白、前清蛋白的水平及其临床意义。方法选择食管癌患者160例为食管癌组,其中早期（Ⅰ-Ⅱ期）82例、晚期（Ⅲ-Ⅳ期）78例;同期选择体检健康者140例为对照组。测定所有研究对象血清IL-8、IL-10、IL-18、C反应蛋白、转铁蛋白、前清蛋白水平。结果 食管癌组血清IL-8、IL-18、C反应蛋白水平高于对照组（P〈0.05）,肿瘤分期越高,IL-8、IL-18、C反应蛋白水平越高（P〈0.05）,且有转移者水平高于无转移者（P〈0.05）;食管癌组血清IL-10、转铁蛋白、前清蛋白水平明显低于对照组（P〈0.05）,肿瘤分期越高,IL-10、转铁蛋白、前清蛋白水平越低（P〈0.05）,且有转移者水平低于无转移者（P〈0.05）。结论 食管癌患者的血清白细胞介素和急性时相反应蛋白水平变化能够反映病情变化,有助于食管癌的临床分期。Objective To investigate the serum levels of interleukin-8（ IL-8）,interleukin-10（ IL-10）,interleukin-18（ IL-18） and C-reactive protein（ CRP）,transferrin and prealbumin in patients with esophageal cancer and their clinical significance. Methods one hundred and sixty patients with esophageal cancer were selected as esophageal cancer group,including 82 cases of early stage（ stage Ⅰ toⅡ） and 78 cases of advanced stage（ stage Ⅲ to Ⅳ）. And 140 healthy individuals during the same period were enrolled as control group.The serum levels of IL-8,IL-10,IL-18,CRP,transferrin and prealbumin were detected in all subjects. Results The serum levels of IL-8,IL-18 and CRP in the esophageal cancer group were significantly higher than those in the control group（ P〈0. 05）. And the levels of IL-8,IL-18 and CRP were higher with the increase of the tumor stage（ P〈0. 05）. The levels of IL-8,IL-18 and CRP in the patients with tumor metastasis were higher than those in the patients without tumor metastasis（ P〈0. 05）. The serum levels of IL-10,transferrin and prealbumin in the esophageal cancer group were significantly lower than those in the control group（ P〈0. 05）. And the levels of IL-10,transferrin and prealbumin were lower with the increase of the tumor stage（ P〈0. 05）. The levels of IL-10,transferrin and prealbumin in the patients with tumor metastasis were lower than those in the patients without tumor metastasis（ P〈0. 05）. Conclusion The changes of serum interleukins and acute phase reaction proteins in patients with esophageal cancer can reflect the changes of the disease,and is helpful for the clinical staging of esophageal cancer.
