摘 要:目的探讨奥沙利铂并上腹部单次大剂量X线照射对大鼠肾组织及功能的影响。方法将80只雄性清洁级Wistar大鼠随机分为化疗组、放疗组、同步放化疗组、对照组,每组20只大鼠。化疗组大鼠腹腔注射奥沙利铂15 mg/kg;放疗组大鼠单次以大剂量6 MV X线12 Gy照射上腹部;同步放化组腹腔注射奥沙利铂15 mg/kg,并以6 MV X线12 Gy单次大剂量照射;对照组大鼠腹腔注射等剂量生理盐水及假照射,在照射后的第3天、1周、2周、3周、6周观察各组大鼠肾组织的改变,同时检测血清肌酐(Scr)、尿素氮(BUN)的变化,免疫组织化学法检测大鼠肾脏组织B淋巴细胞瘤-2(Bcl-2)、Bax的表达情况。结果同步放化疗组在照射后的第3天肾脏皮质比例减小,肾小管上皮细胞间隙增大,部分上皮细胞肿胀、坏死,肾小球毛细血管管腔扩大;化疗组和放疗组出现相似的反应,但程度较轻。各组间不同时间血清Scr及BUN水平比较差异有显著性(F=83.562、754.752;P<0.01),其中在相同时间内同步放化疗组较其他组水平高;各组间不同时间Bax及Bcl-2表达率比较差异有显著意义(F=125.832、215.832;P<0.01),其中在相同时间内同步放化疗组较其他组表达率高。结论同步放化疗组肾脏损伤较单纯化疗组及放疗组严重,其机制可能与抗凋亡基因Bcl-2表达减少,凋亡基因Bax表达增加有关。Objective To explore the effect of Oxaliplatin combined with a single large-dose upper abdominal irradiation on renal tissue and function in rat. Methods A total of 80 male Wistar rats were equally randomized to four groups as: chemotherapy,radiotherapy,chemo-radiotherapy and control groups. The rats in chemotherapy group were given intra-peritoneal injection of Oxaliplatin( 15 mg /kg),those in radiotherapy group received upper abdominal irradiation with 6 MV X-rays at single dose of 12 Gy,those in chemo-radiotherapy group were offered intra-peritoneal injection of Oxaliplatin( 15 mg / kg) and abdominal irradiation with 6 MV X-rays at a single dose of 12 Gy and those in the control group were given equivalent normal saline and fake irradiation. On the 3rd day and in the 1st,2nd,3rd and 6th week after irradiation,the changes of renal tissue were observed,and se-rum BUN and Scr were detected at the same time,the expressions of Bax and Bcl-2 measured as well using immunohistochemical method. Results On day 3 after irradiation,in chemo-radiotherapy group,proportion of cortex decreased,the interspace of renal tubular epithelial cells enlarged,swelling and necrosis were seen in some epithelial cells,and lumen of blood capillary dilatated; similar reactions were seen in chemotherapy group and radiotherapy group,but with lesser extent. The differences in serum levels of Scr and BUN between different time points in each group were significant( F = 83. 562,754. 752; P〈 0. 01),of which,at the same time point,the levels of Scr and BUN in the chemo-radiotherapy group were higher than that in the other groups. The diffe-rences in the expressions of Bax and Bcl-2 at different time points in each group were significant( F = 125. 832,215. 832;P 〈 0. 01),of which,at the same time poin,the expressions of Bax and Bcl-2 in the chemo-radiotherapy group were higher than that in the other groups. Conclusion Renal damage in chemo-radiotherapy group is more seriuos than that in radiotherapy and chemotherapy groups,it
