血清AEP水平检测在脑胶质瘤诊断中的临床意义

摘　　要：目的探讨血清天冬酰胺内肽酶水平检测对于脑胶质瘤诊断的临床意义。方法选取初次诊断的脑胶质瘤患者74例及正常对照患者30例，于治疗前提取血清，用ELISA法测定血清AEP浓度。结果通过统计学分析来推测血清天冬酰胺内肽酶水平在脑胶质瘤诊断中的作用。结果对照组与胶质瘤组、低级别与高级别胶质瘤组、对照组与低级别胶质瘤组、对照组与高级别胶质瘤组血清AEP水平均有显著性差异（P〈0．05），ROC曲线分析AEP诊断脑胶质瘤组和正常对照组时，曲线下面积（areaundercurve，AUC）为0．946，临界值161．08ng／ml，敏感度85．1％，特异度93．3％。血清AEP水平的高低是脑胶质瘤发生的危险因素，OR值为80．18（95Vo可信区间为16．66~385．80）。结论血清AEP水平可作为诊断脑胶质瘤的肿瘤标志物，对于胶质瘤的诊断有高度敏感性和特异性，可以作为胶质瘤临床诊断的方法之一。Objective To investigate the clinical value of serum AEP level in the diagnosis of cerebral glioma. Methods Serum was collected from normal controls （n = 30） and patients with cerebral glioma at primary diagnosis before treating（n= 74）. ELISA was performed to meas- ure the concentration of serum AEP. Results were statistically analyzed to determine the value of serum AEP in the diagnosis of cerebral glioma. Results Results showed that there was significant difference between normal controls and glioma patients, normal controls and low--grade glioma patients, normal controls and high--grade glioma patients, low--grade glioma patients and high-- grade glioma patients（P〈0.05）. A threshold expression value （161.08ng/ml） was chosen from receiver operator characteristic curves （ROC） ,and the level of serum AEP was a risk factor of gli- oma（OR= 80. 18,95 % CI= 16.66-385.80）. Conclusion Serum AEP is a promising diagnostic tumor biomarker in the diagnosis of cerebral glioma with high sensitivity and specificity.
