单孔双管与单孔单管治疗慢性硬膜下血肿的对比研究

摘　　要：目的探讨单孔单管与单孔双管治疗慢性硬膜下血肿的临床疗效。方法选取我院2012年1月～2016年1月慢性硬膜下血肿患者104例，随机分观察组对照组，每组各52例；观察组采用单骨孔双管治疗，对照组采用单骨孔单管治疗，观察两组患者手术时间，住院时间，及血肿清除情况，术后复查头颅CT了解术后残余量及血肿腔残气，3月后血肿复发情况。结果两组患者在颅内积气方面比较，观察组明显优于对照组，差异具有统计学意义（P〈0．05）；血肿完全清除率和术后残余量比较，观察组明显优于对照组，差异具有统计学意义（P〈0．05）；住院时间和手术时间方面比较，差异不具有统计学意义（P〉0．05）。3个月复查，观察组患者复发率明显低于对照组，差异具有统计学意义（P〈0．05）；住院时间和手术时间方面比较，差异不具有统计学意义（P〉0．05）。结论单骨孔双管技术治疗慢性硬膜下血肿疗效显著，血肿清除率高，复发低率，可作为外科治疗慢性硬膜下血肿的一种较理想的手术方式。Objective To investigate the efficacy of single--hole with double pipe treats chronic subdural hematoma. Methods 104 cases of chronic subdural hematoma patients were col- lected and randomize divided into two groups：the control group and the observation group, 52 ca- ses in each group. Patients in observation group receive single hole with double pipe therapy,pa- tients in the control group receive a single bone hole with single pipe treatment. The data of sur- gery time,hospital stay,and hematoma hematoma recurrence were collected in the two groups af- ter operation, and use the CT to evaluate the amount of residual gas and hematoma. After 3 months,patients receive the CT examination to evaluate the recurrence. Results Intracranial gas in observation group is less than the control group, the difference was statistically significant （P 〈0.05）; the clearance rate of hematoma and residual amount in observation group is less than the control group,the difference was statistically significance （P〈0.05）; after 3 months,3 pa- tients in control group and 0 patient in observation group were hematoma recurrence, the differ- ence was statistically significance （P〈0. 05）. The hospital stay and the operation time in two groups is same,and the difference was not statistically significant （P〉0.05）. Conclusion The single hole with double pipe for treatment of chronic subdural hematoma is good,low recurrence rate,and the surgical treatment can be used to treat chronic subdural hematoma.
