胰腺实性假乳头状瘤的MSCT及临床特点探讨

摘　　要：目的：探讨胰腺实性假乳头状瘤（ SPTP）的MSCT表现特征，提高MSCT对该病的诊断正确率。方法回顾性分析21例经手术病理证实为SPTP患者的临床、病理资料及MSCT平扫、强化的特点。结果21例SPTP患者中胰头部发病9例，胰腺体部2例，胰尾部发病10例；直径≤5 cm者5例，﹥5 cm者16例；CT表现为单发体积较大的囊实性肿块7例，实性肿块8例，囊性肿块6例；平扫呈等、低密度，边界清晰，内可见出血及条状分隔，有薄包膜，瘤体及包膜可以有钙化，增强扫描实性部分及纤维间隔动脉期轻中度强化，门脉期及延迟期持续强化，包膜强化程度低于胰腺正常组织。病理示良性17例，低度恶性4例。结论 SPTP的CT表现具有一定的特征性，综合分析临床资料及CT表现对该病的诊断及鉴别诊断具有重要价值。Objective To investigate the MSCT features of pseudo papillary tumor of the pancreas（ SPTP）,and to improve the diagnostic accuracy of MSCT. Methods To retrospectively analyze the clinical,pathological data and MSCT plain scan,enhanced features of 21 patients with pathologically confirmed SPTP. Results In 21 cases of SPTP,9 cases had pancreatic head disease,2 cases pancreatic body,10 cases pancreatic tail. The diameter≤5 cm in 5 cases,﹥ 5 cm in 16 cases. CT showed solita-ry large volume cystic solid mass in 7 cases,solid mass in 8 cases,cystic mass in 6 cases. Plain CT scan showed low density,boundary clear,visible hemorrhage and strip separated with a thin capsule,tumor and capsule can have calcification,enhancement of solid and fiber septal artery with mild to moderate en-hancement,portal venous phase and delayed phase,capsular enhancement,cystic part no enhancement, strengthen the degree was lower than normal pancreatic tissue. Pathology showed 17 cases benign,4 ca-ses of low grade malignancy. Conclusions CT SPTP has characteristics,comprehensive analysis of clin-ical data and CT performance has important value for diagnosing and identifying this disease.
