摘 要:目的:研究壮筋续骨汤对骨折大鼠骨痂组织降钙素受体(CTR)、核心结合因子α1(CBFα1)表达的影响。方法:成年健康雄性Wistar大鼠60只,用线锯切断股骨中段制备骨折模型,以壮筋续骨汤水煎液灌胃治疗。苏木精-伊红染色观察骨痂组织病理变化,免疫组织化学染色检测骨痂组织中CTR和CBFα1的表达量。结果:骨折断端在骨折第7天主要为炎性细胞及肉芽组织所填充,第14天为纤维性骨痂组织,第21天为软骨性骨痂,第28天有大量骨小梁形成。骨折后7-21d,对照组与实验组骨痂组织CTR表达逐渐增多(t=2.428,3.003;P=0.008,0.027);第28天时对照组表达显著减少(t=2.816,P=0.012),实验组表达保持较高水平且显著高于对照组(t=3.042,P=0.01)。骨折后7-28d骨痂组织中CBFα1表达一直保持在较高水平,实验组各时间段的表达强度均显著高于对照组(t=2.598,2.639,3.128,3.679;P=0.003,0.009,0.022,0.023)。结论:壮筋续骨汤可能通过上调骨痂组织中CBFα1的表达而促进骨折愈合,通过调节破骨细胞的功能状态促进软骨性骨痂的软骨内骨化和重塑。Objective: To detect the effect of Zhuangjinxugu (ZJXG) decoction on the expression of calcitonin receptor (CTR) and core binding factor α1 (CBFal) in callus of femur fracture rats, and investigate its mechanism of promoting fracture healing. Methods: Total of 60 healthy adult male Wistar rats were subjected to establish the femur fracture models by breaking off the femur at middle point with a wire, and they were gavaged with ZJXG decoction. The fracture healing process was analyzed by hematoxtlin-eosin (HE) staining. The expression of CTR and CBFα1 in callus were evaluated by immunohistochemical staining. Results:The femur fracture-end was filled with inflammatory cells and granulation tissue, fibrous callus,cartilaginous callus at 7th,14th,21st days after fracture,and the osseous callus was formed by a large number of bone trabecula at 28th days after fracture. The expression of CTR in calius was increased significantly from 7 days to 21 days (t=2.428 and 3. 003 , P= 0. 008 and 0.027) both in control group and treatment group,but decreased at 28 days after fracture in control group (t = 2. 816, P = 0. 012), which were significantly lower than treatment group (t = 3. 042,P=0.01). The expression of CBFα1 in callus were kept a high level from 7 days to 28 days both in control group and treatment group, and its expression in treatment group was significantly higher than that in control group at the every corresponding time points (t=2. 598,2. 639,3. 128,3. 679 ; P=0. 003,0. 009,0. 022,0. 023). Conclusion:ZJXG decoction can promote fracture healing by up-regulating the expression of CBFal in the callus tissue. It also can regulate the function of osteoclast to promote the endochondral ossification process in cartilaginous callus and remodeling process in the cartilage.
