摘 要:目的探讨胆囊癌组织中细胞周期检测点激酶1(Chk1)的表达及其与胆囊癌患者临床病理因素和预后的关系。方法总结2009年6月至2012年12月间收治的64例行根治性手术的胆囊癌患者的临床及预后资料。采用免疫组织化学染色法检测胆囊癌、癌旁及胆囊炎组织中Chk1的表达情况,分析其与胆囊癌患者临床病理因素及预后之间的关系。结果 Chk1在胆囊癌组织中的表达主要位于细胞质。在胆囊癌、癌旁、胆囊炎中表达的阳性率分别为60.94%(39/64)、47.36%(9/19)、15.38%(2/13),Chk1在胆囊癌组织中的表达明显高于癌旁及胆囊炎组织(P<0.05)。Chk1的表达在不同胆囊癌组织类型、分化程度、淋巴转移和Nevin分期中差异有统计学意义(P<0.05)。Chk1表达阳性和阴性的胆囊癌患者的中位生存时间分别为9.8个月和26.6个月,生存分析显示Chk1表达阳性患者的1、3、5年生存率分别为27%,16%,7%,相对于阴性表达明显降低,差异有统计学差异(P<0.05)。单因素和多因素分析结果显示:Chk1阳性表达是影响胆囊癌患者预后的危险因素(P<0.05)。结论胆囊癌组织中Chk1高表达,与胆囊癌的侵袭转移相关,是影响胆囊癌患者预后的危险因素。Objective To explore the expression of Chk1 in the gallbladder cancer(GBC)and its relationship between clinicopathologi-cal features and prognosis of patients. Methods We retrospectively summarized the clinical data of 64 GBC patients who performed radical surgery in the Department of Hepatobiliary Surgery at the First Affiliated Hospital of the Xi′an Jiaotong University from June 2009 to September 2012. The GBC tissue specimens from surgical resection were collected. The expressions of Chk1 in GBC tissues,pericarcinoma tissues and cholecystitis were examined by the immunohistochemistry staining,and their relationship with clinicopathological factors and prognosis of patients was analyzed. Re-sults The expression of Chk1 in GBC was predominantly detected in the cytoplasm of the tumor cells. The positive rates of Chk1 in GBC tissues, pericarcinoma tissues and cholecystitis were 60. 94%(39 / 64),47. 36%(9 / 19),and 15. 38%(2 / 13),respectively,with a significant differ-ence( P 〈 0. 01). The difference of Chk1 expression was statistically significant in in different GBC tissue types,differentiation,lymph node me-tastasis and Nevin staging( P 〈 0. 05). The median survival time of Chk1 positive expression and negative expression in GBC was 9. 8 months and 26. 6 months. Survival analysis showed that 1,3,5 - year survival rate of patients with Chk1 positive expression were 27% ,16% ,7% ,with re-spect to the negative expression was significantly reduced survival time,which had statistically significant difference( P 〈 0. 05). Conclusion The increasing expression of Chk1 can be observed in GBC tissues. It′s associated with the invasion and metastasis of GBC. The increasing expres-sion of Chk1 is closely correlated to the GBC prognosis and maybe an independent risk factor affecting the prognosis of the GBC patients.
