肝细胞肝癌患者血清HMGB1的表达及其临床意义

摘　　要：目的研究肝细胞肝癌(HCC)患者血清高迁移率族蛋白B1(HMGB1)水平变化及临床意义。方法应用酶联免疫吸附试验定量检测HCC患者(A组,32例)、肝良性病变患者(B组,28例)和健康对照者(C组,30例)术前血清HMGB1水平,分析HCC组血清HMGB1与临床病理特征的关系。绘制受试者工作特征曲线(ROC曲线),评估HMGB1对HCC的诊断价值。结果 A组和B组血清HMGB1水平分别为(24.6±5.7)ng/ml和(13.8±3.2)ng/ml,高于C组的(5.6±1.7)ng/ml(P<0.01)。A组血清HMGB1水平与血清甲胎蛋白(AFP)水平、TNM分期和淋巴结远处转移密切相关(P<0.01),但与年龄、性别、肿瘤大小和分化程度无明显相关性(P>0.05)。A组血清HMGB1与AFP呈正相关(r=0.642,P<0.01);其血清HMGB1和AFP的ROC曲线下面积分别为0.714和0.865(P<0.01)。结论 HCC患者血清HMGB1水平明显升高,可望成为诊断HCC的一项指标。Objective To investigate the expression and clinical significance of serum high mobility group box I（HMGB1） in the patients with hepatocellular carcinoma（HCC）. Methods Serum levels of HMGB1 were detected by ELISA in the patients with HCC（group A, 32 cases） or benign liver diseases （28 cases）, and healthy people （group C, 30 cases）. The association of HMGB1 expression and elinicopathologic features of HCC was analyzed. The value of HMGB1 expression in the diagnosis of HCC was evaluated by drawing the receiver operating characteristics（ROC） curve. Results Serum levels of HMGB1 were higher in groups of A and B than those in group C[（24.6± 5.7） ng/ml and （13.8±3.2） ng/ml vs. （5.6±1.7） ng/ml] （P〈0.01）. Serum HMGB1 levels were closely related with alpha-fetus protein（AFP） concentration,TNM staging and lymph node metastasis （P〈0.01） ,which had no significant relation with the age, sex, tumor size and tumor differentiation （P〉0. 05）. Serum level of HMGB1 was positively correlated to AFP concentration （r = 0. 642, P〈0. 01）. The areas under ROC curves were 0. 714 for HMGB1 and 0. 865 for AFP（P〈0. 01）. Conclusion Serum HMGB1 level is significantly increased in the patients with HCC, which could be as a potential marker for the diagnosis of HCC.
